INJURED STATES HE WAS SHOVELING LOOSE MATERIAL FROM AROUND THE #293 BELT TAIL ROLLER AND THE SHOVEL CAME INTO CONTACT WITH THE MOVING ROLLER WHICH CAUSED THE SHOVEL TO KICK BACK AND STRIKE HIS LEFT THUMB. THE RESULTING INJURY WAS A SIMPLE FRACTURE OF THE LEFT THUMB LOCATED JUST BEHIND THE THUMB NAIL.

THE MANTRI P PULLED FORWARD FORCING THE SCOOTER INTO INJURED, PINNING INJURED BETWEEN THE LOCKERS & THE FRONT OF THE SCOOTER.
EMPLOYEE WAS WALKING BESIDE THE #8319-MBC THAT WAS POSITIONED CLOSE TO THE MINE RIB. THE #8320-MBC OPERATOR BACKED HIS UNIT UP CAUSING THE #8319-MBC TO SWING TO THE RIGHT, MASHING EMPLOYEE'S L EFT LEG BETWEEN FRONT OF #8319-MBC AND THE MINE RIB.

HE TRAMMED THIS UNIT OVER A LARGE ROCK THAT HA DFALLEN ON THE FLOOR, WHENT HE TRACK MOVED FORWAR D OFF THE ROCK, THE OPERATORS COMPARTMENT STRUCK THE ROCK, FORCING THE OPERATORS COMAPRTMENT UP, CAUSING HIS HEAD TO COME INTO CONTACT WITH THE MINE ROOF.

EE WAS WATCHING THE TRAILING CABLE ON HELIMINER 8233 WHILE OTHER EE MOVED THE MACHINE. EE SWUNG THE MACHING AROUND AND THE CONVEYOR BOOM STRUCK EE AND MASHED HIM AGAINST THE MINE RIB.

HE COULD NOT GET STOPPED A ND COULD NOT TURN CORNER. THE LEFT FRONT & LEFT SIDE OF SCOOTER HIT THE RIB. EMPLOYEE'S LEFT HIP HIT RIB CAUSING INJURY.
AT START OF 3RD SHIFT ON MARCH 6 1989 WHILE IN ROUTE TO THEIR ASSIGNED WORK AREAS THEY STOPPED AT 140 SHOP TO PICK UP TOOLS & SUPPLIES WHILE BACKING UP MANTRIP TO TURN AROUND AT LEAST ONE OF T HE PASSENGERS WITHOUT REALIZING SHINED THEIR MINE LIGHT INTO MANTRIP OPERATORS EYES BEFORE MANTRIP COULD BE STOPPED BACK OF MANTRIP HIT THE RIB
EMPLOYEE WAS PLACING WOODEN BLOCKS BETWEEN THE TAIL SECTION OF A CONVEYOR BELT AND THE BELT FEEDER. THE BELT FEEDER MOVED SUDDENLY CATCHING HIS RIGHT THUMB BETWEEN THE BELT FEEDER AND THE WOO DEN BLOCKS.
THE INJURED WAS3CLEANING PRODUCT FROM WIPER ON HEAD ROLLER OF OVERSIZE PRODEUCT BELT,WITH APUNCH WHILE BELT WAS RUNNING.THE PUNCH BETWEEN WIPER AND BELT ON HEAD ROLLER,PULLING FINGERS OF RIGHT HAND INTO THE 1INCH POINT.AMPUTATING THE FIRST JOINT OF LITTLE AND TWO MIDDLE FINGERS OF RIGHT HAND.
EMPLOYEE WAS RI8ING ON GOPHER TRUCK,WATCHING CABLE THAT WAS BEING MOVED.THE DRIVER STOPPED TRUCK SUDDENLY.EMPLOYEE MOVED HIS ARM SUDDENLY TO BRACE HIMSELF AND HE RECEIVED A SPRAIN TO HIS RIGHT SHOULDER. 3
