MINER WAS INSTALLING VENT LINE OFF THE 2 YARD LOADER, MINER WAS STANDING ON THE FRONT PART OF THE LOADER ON THE DOWNHILL SIDE OF THE VENT LINE. THE LOADER PARK BRAKE WAS SET AND TRANSMISSION W AS IN FIRST GEAR REVERSE POSITION. MECHNIC WAS WORKING ON ELECTRICAL WIRING ON LOADER. THE INSPECTION DOOR WAS OPEN WITH LOADER RUNNING. THE VIBRATION OF THE ENGINE CAUSED THE DOOR TO COME DOWN ON MECHNICS HAND.
