Showing all 18While driving a kubota utv, some salt dust fell in eyes and when they went to wipe the dust from their eyes, they swerved to the right and drove into a chunk of salt and rolled the utv onto it's side. When they rolled the UTV, their mine belt had a harness ring on the back and that's what they landed on, fracturing their L2 vertebrae.
Employee was trying to empty a wheel barrel of salt into a bucket of a frontend loader. Operator of frontend loader rolled the bucket of loader to be flat on the ground. Injured employee did not realize that their foot would be under the bucket. Bucket pinched foot, causing a contusion. Employee started losing time on 10/11/2022.
Employee was traveling to a work area in a Polaris ranger with their foot outside of the cab. Employee's foot hit the rib causing a broken ankle.
When driving Maintenance Vehicle back from mining section, vehicle hit a chunk of salt in roadway. Steering wheel turned fast and caused strain on right shoulder and arm. Arm kept bothering employee. Went to doctor, and doctor wants to do surgery. Started losing time from work on 11/13/2017.
Employee was driving LHD Loader from 3 Yard East to 4 Yard North when he was jarred in the loader. Employee injured neck. Lost time.
While riding in a maintenance vehicle, the unit hit a uneven undercut on the mine floor jarring the pick-up truck. The jarring action caused employee to have a pain in the lower back.
Employee was operating water wagon and tractor employee slipped off of the seat of tractor and landed on his back.
Miner was operating loader Underground on 11/29/2012 he told the supervisor he wanted to go home two hours into his shift when asked if he was sick or hurt he replied "No". The next day the employee (Miner) returned to work with a Doctors note saying he had a Strain, Neck Spinal Cord injury.
Employee was driving John Deere Gator, went through strip door too fast. Strip of door caught front of gator rolling over gator. Employee bruised hip & back, causing lost time.

When the bucket was dumped, a chunk of salt rolled down the pile next to the stamler towards the employee. The employee tried to get out of the way but slipped. EE was helping install channels for an extension onto a conveyor with a forklift. One channel slipped off of the forks of the forklift and pinched three fingers of EE's right hand, causing fractures. EE has been given light/restricted duty.
Employee was operating Elphinstone LHD #5 when the LHD went over a jump in the floor causing the operator to strike his head on the roof of the canopy causing an injury to the neck. Employee was given light duty.
Employee states that he hurt his back while driving Getman Lift Truck on or aboout December 1, 2004. Pain grew worse in his back over past few weeks until employee finally went to the Doctor of 01/05/05. Employee received light duty for three weeks.
EE WAS CHANGING A CYLINDER ON AN UNDERCUTTER, WHEN THE CYLINDER WAS LIFTED WITH A FORK LIFT, THE EE'S LEFT RING FINGER TIP WAS PINCHED BETWEEN THE UNDER CUTTER FRAME & THE CYLINDER. TIP OF FIN GER HAS A HAIRLINE FRACTURE.
