The employee was the passenger of a gravely (UTV) riding along when the vehicle had a minor backing incident into a wooden post. The driver of the vehicle was fine but the passenger stated had a headache after the incident and requested to go home.

Employee was driving haul unit loaded with slurry up a ramp. Tires began to spin and lose traction on the slick ground. Unit began to slide backward then tip back onto its end gate causing the unit to sharply jolt backward causing pain in employees neck and back.

Employee was working inside the Tire Changing Shop when a pickup truck crashed through the wall of the shop. He was struck by the rear tailgate in his upper extremities (arms) and thrown back against his desk. He was treated for sprain injuries.

Employee was steering disabled haul unit being towed by a loader over railroad tracks because its air system failed and back brakes locked up. Unit's bed was in raised position. When haul unit had cleared tracks both units stopped. Sudden stop caused haul unit's bed to drop. The shock jolted employee causing multiple strain injuries and pain; even with seat belt fastened

Rocks were stuck between the top of the frame of the bed and the endgate. Employee tried to raise the endgate up with both hands. All of the sudden, the endgate swung closed. His right ring finger got pinched between the endgate and the frame.

When he got to the intersection with the main haul road, he turned his unit left and collided with another approaching haul unit. Treated at ER. Released same day with minor injuries. Haul unit received major cosmetic damage.

Employee was digging the floor of the pit with the Loader. He hit a hard spot with the bucket and the Loader came to a sudden stop. Employee jolted forward and his seat belt restrained him. Felt pain in back. Requested medical treatment on 03/26/10.

The employee was checking the end dump tailgate for material. There was a rock blocking the tailgate, keeping it from closing. He grabbed the rock with his left hand to try to loosen it. The rock came loose and the tailgate shut, pinching the tip of his left index finger and tip of left thumb. He was wearing gloves.

EMPLOYEE WAS OPERATING A 35-TON HAUL TRUCK. EMPLOYEE STRAINED HIS BACK WHEN THE FRONT OF THE TRUCK WAS LIFTED, THEN DROPPED. THE TRUCK WAS LIFTED BY THE WEIGHT OF THE ROCKS THAT HAD STUCK IN THE TAILGATE WHEN THE EMPLOYEE RAISED THE BED OF THE TRUCK TO DUMP ROCK FROM THE BED INTO A HOPPER. EMPLOYEE WAS RELEASED TO WORK WITH WORK RESTRICTIONS.

EE CLAIMS THAT HE BEGAN HAVING LOWER BACK PAINS AFTER HE GOT HOME AFTER HE HAD DRIVEN A 1998 CASE FRONT END LOADER MODEL 621B ALL DAY. HE CONTINUED TO OPERATE THIS MACHINE DAILY UNTIL 4-3-02 W HEN THE PAIN BECAME UNBEARABLE. HE WAS TREATED TAT PRO MED CLINIC AND RESTRICTED FROM DUTY UNTIL 4/8/02 AT WHICH TIME HE WAS RELEASED TO FULL DUTY.

EE CLAIMS THAT WHILE DRIVING A HAUL UNIT UP A RAMP FROM A SLURRY POND, THE LEFT REAR TIRE HIT A HOLE IN THE GROUND, CAUSING THE FRONT OF THE HAUL UNIT TO COME OFF THE GROUND AND WHEN IT CAME B ACK DOWN, HE FELT A BURNING PAIN TO HIS LOWER BACK. HE WAS SEEN BY HIS PERSONAL PHYSICAN ON 7-12-01 AND EXCUSED FROM DUTY UNTIL 7-15-01.

EE CLAIMS THAT HIS BACK HAS BEEN BOTHERING HIM FOR THE LAST FEW MONTHS AND THAT AT APPROX. 4:00 PM ON 2-20-00, HE STARTED FEELING SEVERE PAIN IN HIS LOWER BACK AND LEG & ON TUE. 2-15-00 HE DEC IDED TO SEE HIS DR. HE WAS DIAGNOSED WITH A HERNIATED DISK. HE CLAIMS THAT THIS WAS AS A RESULT OF DRIVING A HAUL TRUCK FOR MANY YEARS. HE WAS EXCUSED FROM DUTY.
