Employee was hauling rock to the crusher under normal conditions. EE was traveling up a ramp from level two to level one and EE's truck stopped on the ramp. EE tried to back down the ramp and lost control of the vehicle resulting in the truck turning over on its side.

An off duty EE was driving in a restricted work zone when he drove his car under a front end loader who was backing up,
WHILE LOADING LOWER ROW OF DRILL HOLES IN HEADERTHE SUPPORT BRACKET FOR THE POWDER TRUCK MANLIFTBROKE CAUSING THE BASKET TO TILT. EE FELL OFF BASKET AND STRUCK HIS HEAD ON FLOOR. THE FALL CAUS ED A CONTUSION TO HIS HEAD REQUIRING 8 SUTURES.
EMPLOYEE WAS LOADING AND HAULING MATERIAL TO THE JAW CRUSHER. EMPLOYEE WAS DRIVING R50 EUCLID. CLAIMS THAT THE BREAK THREW FELL AND HIT THE BACK OF HIS TRUCK ND STRAINED HIS NECK AND BACK.
