EE was driving a rock truck down a hill toward the Cedar Grove deep mine when his a rough spot in the road. Foggy conditions was present.
EE loaded a rock in his bucket which was too large for his loader. The loader tipped forward and the rock fell out, letting the loader hit the ground. EE was not wearing his seat belt and was bounced out of his seat, striking his arm against the hand bar on the dash.
