Employee was wiping down the inside of his loader. When he was cleaning aroung the air vents in the overhead A/C unit, his right hand came in contatct with the A/C fan. He was cut on his middle and ring fingers (the A/C unit was running at the time).
RAIN HAD CAUSED HAUL ROAD DETERIORATION. THE RAIN ALSO CAUSED POOLING OF WATER IN THE LOADING AREA. THE HAUL TRUCKS HAVE DRAIN HOLES IN THEIR BEDS, SPEWING ADDITIONAL WATER ON THE ROAD. EE SAI D HIS NECK & LEFT SHOULDER WERE STIFF & SORE FROM OPERATING THE HAUL TRUCK IN THESE CONDITIONS.
PULLING UP TO THE LOADER IN THE PIT TO GET LOADED & DID NOT SEE THE LARGE ROCK & RAN OVER IT WITH BACK TIRES OF THE HAUL TRUCK WHICH JARRED THE HAUL TRUCK & THE OPERATOR.
