3 contractor employees were on site wiring in a recently set up wash plant in Cedar City, UT. One of their employees was climbing toward the head pulley on the conveyor. The other two employees were on the ground feeding the employee wire. The legs that suspend the conveyor in the air collapsed causing the conveyor to fall with ACM employee on top of conveyor.

Attempting to get rock unstuck EE was reaching into catch box & got EE's hand stuck between catch box and head pulley of the slinger belt. EE leaned forward with the rock as it fell. The rotation of the head pulley pulled EE's hand and the rock into the gap between the head pulley and the catch box. Partial amputation of EE's Right Pinky Finger.

EMPLOYEE WAS OPERATING A LOADER, HE HIT A ROCK OR BUMP & IT CAUSED HIM TO HIT HIS LEFT RIB CAGE ON THE ARMREST OF HIS SEAT. THE IMPACT FRACTURED A RIB ON HIS LEFT SIDE. HE WAS WEARING HIS SEATBELT.
