The employee was assisting another employee reconnect a hoist jack to boom on a 988 Cat loader. The employee was holding the jack in line, while the other employee was lifting the jack with a service crane. The chain slipped allowing the crane rope to swing. The winch block struck the employee in the head resulting in a laceration. Treated at E.R.

EMPLOYEE WAS DRIVING A HAUL TRUCK. WHILE HE WAS BEING LOADED A ROCK FELL FROM THE LOADER BUCKET & SPLASHED MUD INTO HIS LEFT EYE. EE WAS TAKEN TO THE DR. WHERE THEY FLUSHED HIS EYE & HE RETURNED TO WORK.
