Ee was inspecting the end of a parked Genco mantrip, when it rolled forward and Ee was caught between vehicle and tool cart. Ee was seen at local ER and released to full duty. The next day the Ee was experiencing some pain and was taken to local clinic. Dr. advised Ee to stay home the next shift, and return on next regular shift after weekend with no further restrictions.

Showing all 29Ee was driving a mantrip in the belt room, and encountered a lower section of back Ee struck hard hat and head on back causing a laceration to the top of scalp which required staples to close the wound.
EE contacted a low-hanging salt water formation with hard hat while driving into the UG part of mine. Flesh wound was cleaned. No medical treatment & released to full duty. On 4/15, EE experienced a seizure at home. WC determined it was related to the 2/24 incident resulting in a reportable injury. Diagnosed head trauma resulting in post-traumatic epilepsy. EE remains hospitalized.
Ee was driving a mantrip, and as traveling through an air curtain the Left front tire climbed the rib. Ee's Right knee was impacted and caused a fracture.
Operating Genco truck pulling trailer with 2 MT totes. Genco struck rock and EE lost control of steering. Genco rode up on muck and overturned. EE extricated self immediately. Abrasions and bruises--non-displaced fracture R ulnar styloid.
EE operating transloader to load truck from rail car. EE stepped too close to conveyor while attempting to close car gate. R leg was caught between the belt and guide tire. E-Stop bar activated to stop belt. EE extricated in < 30-minutes.
On 6/15, EE reported recurring back pain from an off-job back strain injury. By 7/8, short-term disability was denied and EE subsequently claimed work injury from operating SC on rough haulage in S-812. After investigation, claim was accepted. LWD start 6/16.
EE working OT shift leaving early is believed to have blacked out because of a previously un-diagnosed medical condition. EE driving TomCar solo and struck unattended lube wagon. Collision resulted in fracture R tibia
EE was driving a diesel TomCar to production area 904. EE fell asleep & veered into adjoining conveyor (13ML). Struck L forearm on belt structure and forehead on E-stop/alarm J-box. Lacerations required stitches
Operator slipped while attempting to reach e-stip on lexiveyor. L-foot slid in front of tire and was run over resulting in L ankle sprain.
EE reports: tramming loaded SC to feeder and struck head on low-hanging belt clip. Resulted in neck strain. Treated as FA and under observation while performing required job duties. Pain migrated to lower back resulting in LWD beginning 06/14/2013.
EE cleaning muck near conveyor underpass. Vehicle towing empty trailer passed under underpass. EE stood up and greeted vehicle driver when she was struck and run over by empty trailer. Bruising to both legs--sprain R knee. Temp xfer pending recovery
EE engaged in picking wood from trash pile. Endloader approx. 50' away moving a section of pipe. A second section of pipe under tension released from pile & struck EE L hand causing laceration requiring stitches.
EE was operating shuttle car and hit a bump. EE bumped against inside of cab bruising L ribs and shoulder. EE was treated and released to perform normal work duties. Subsequent medical evaluation generated restrictions and resulted in lost-time 5/5/10.
EE was driving joy shuttle car to the miner and hit his head on roof top, causing him to fall out of the car and land on his shoulder. His first day of work under restrictions is Oct. 15, 2009
EE was driving under a belt cross while pulling a fan with a toolcat. There was loose muck on the floor and he lost traction. He backed up to get better traction. When he was about halfway through, the front tires caught loose muck, which drove Employees head between a return roller and the belt.
EE had just finished sitting down on the Tomcar and felt a discomfort on his lower back and as he proceeded to his work area the pain worsened.
EE was driving a joy car which has two seats each seat facing different directions of travel. While swinging his body from one seat to the other he heard a pop on his right knee and felt some pain. EE did not seek medical attention until 5/27/2008. At that time Restrictions were issued.
Witness was driving 007 tom car out of the work area and employee was the passenger. He encountered some dust by drive section of 150 and continued to drive thru it but missjudged the turn off and hit a piller block. Employee hit his right knee on the front frame of the passenger seat, causing a laceration to his knee. Required stitches, no lost time, no restrictions.
Employee was running 71 car in low back and hit his head.
Employee was driving 176 ram car with a load going to the feeder when the articulation broke. The sudden stop caused the employee to strike his shoulder against the inside surface of the cab.
Employee was pushing a transformer with a Getman utility vehicle. As he was using the front of the vehicle to do so he had his right foot positioned outside the cab. While pushing, the vehicle slipped off the transformer causing the vehicle to drive up next to the transformer pinching his right small toe between the vehicle and transformer.
EE was driving down S400 belt line looking for wood blocks. While looking to the left side of the room, he drove up onto a heaved floor and either struck his head or was struck by something causing a laceration. Stitches were required and missed workdays started on 3/11/07.
Employee was lifting steel grating on the forks of forklift when grating fell on employee's lower RT leg, causing a cut that required stitches.
EMP & CO-WORKER WERE PULLING 20 HP FAN CABLE. HE WAS HOLDING NIP WHILE SITTING IN PASSENGER SEAT. AS CAR MOVED, CABLE CAUGHT & PULLED AWAY FROM HAND WEDGING IT ON SIDE BAR LACERATING RT LITTLE FINGER. 8 STITCHES REQUIRED.
EE was driving a S/C and slowed down to make a turn, stepped on tram, it jumped into high tram jerking him forward. He felt pain in lower back.
Employee was driving #999 Jeep to work area. He hit a rough spot in roadway causing a jarring sensation to his back. Asked to see Doctor 2-3-06. Started losing time 2-3-06.
Employee having trouble with lights on ram car. He looked down at switch and ran into the mine rib. Whiplash type injury to neck and back. Started losing time 7-23-05.
Employee was driving out of gob room when he saw a light and glanced over to see who it was. At that time he hit a high spot on the roadway causing his head to hit the back.
