Showing all 13Employee was adjusting the forks on a skidsteer unit to be able to pick up scrap. As he reached between the backing plate and the mast to unlock pin holding forks in place the operator tilted the forks back pinning his arm between backing plate and the mast. Employee ended up with a slight fracture and some minor muscle damage on left forearm.
Loading hyd. hand drill in scoop when he felt a pain in his lower back. The drill hoses was hung on a anchor pin. He started missing work on 1/23/2012.
While driving a mine buggy the wheel of the buggy caught a rock, thus spinning the steering wheel which caught employee's left hand. This resulted in a laceration to his left hand requiring stitches.
He was dumping a load of reject half way up the hoist pin broke bed came down with our the help of hoist.
EE was scooping the rib on roadway at the overcast and a roof bolt that was buried in gob, caught scoop bucket and flew back and caught him over the right eye causing a laceration. EE was wearing his safety glasses; the roof bolt broke his safety glasses.
In #5 entry at spad #839 EE was pulling a loose piece of rock off the right corner. The rock come down and pulled the hung miner cable down onto his head, causing pain in lower back.
While backing little two man Johnson buggy up, ran over loose coal and got head in the top and stove neck up. Did not have any medical treatment til 5/5/09 and started missing work on 5/6/09.
He was moving shuttle car for the continuous miner to travel by, when his head struck a low corner. He was taken to the ER and checked out and released that night with strained muscles in the neck and head area.
Standing between roof bolter and 2 man buggy in break between #6#7 entry. A ram car came around block in #6 entry going to Continuous miner hitting 2 man buggy and pushing it into the roof bolter, catching his right leg between the two pieces of equipment. Started missing work on 9/12/08.
Employee was operating a continuous miner in the #6 face, where the mine floor was wet and muddy. He had finished loading a coal hauler and as the hauler was pulling away, it began to spin which allowed the rear of the hauler to slide over, striking EE's right ankle.
Tried to use foot to hold back curtain.
Employee states that he was experiencing pain in his back from a shift earlier and the pain became more extreme after a sudden jolt while riding in the man bus.
