Employee was welding on a chute at the stoker loadout. Employee was in a basket attached to a CAT telehandler. The basket was raised six feet off the ground when it unattached from the telehandler falling to the ground. Employee sustained a fracture to the left tibia.

Showing all 21Helping change ram car batteries. The safety chains were tight, so the car pushed the battery into the rib to take tension off the chains. I.E. reached in to loosen the chains when they became tight again pinching their right index finger between the battery and the safety chain. Caused a fracture and laceration that required stitches to the I.E's right index finger.
Employee was helping another start the face scoop. Employee sat down in the cab and leaned forward to pull on the breaker handle when employee felt a pain in lower back. Through company investigation, information received January 6th, 2023 it was determined this incident need be reported.
At the completion of a job, EE's left arm got caught by a belt roller on the plant feed belt causing abrasions. EE attempted to knock belt skirting down against the belt with a hammer and missed, allowing coat sleeve to be caught between the top roller and the belt. Transported to Springfield Memorial Medical ER for treatment where EE was admitted for observation.
The injured was making an exam as another mantrip was trying to squeeze between the examiner's ride and the rib, catching the ride and bumping it forward. The bumper struck EE's left knee. EE continued working, finished EE's shift and did not seek medical treatment. Became a lost time after receiving a "no work" restriction 8/17/2017 pending surgery to repair a meniscus tear.
While riding in a mantrip was bounced off the seat when the vehicle ran over an unknown object. The injured experienced pain prompting a first aid only report 8/25/2016. Employee continued working without restriction until they began missing work due to surgery 3/23/2017.
While operating a ram car drove over a sulfur ball in the roadway and bounced into the canopy jamming his head and neck. Diagnosed as a neck strain and restricted to no driving of equipment until seen again by his physician.
While operating the Raymond forklift pinched his right little finger between the machine and a shelf causing a laceration that required sutures to close. He was treated and returned to unrestricted duty.
While driving a mantrip along the travelway caught the canopy on low steel causing him to strike his chin leaving a laceration that required sutures to close. Treated and released to unrestricted duty returning to work on his next scheduled shift.
While unloading hi-line from a car 10/11/2010 caught the two small toes of his right foot under a wheel. He continued working not seeking medical attention until 10/13/2010 after swelling and pain persisted. X-rays did not reveal any fractures and he was released to unrestricted duty. Became a Restricted Duty after the site of initial injury became infected 10/27/2010.
While operating a mantrip struck a battery ramcar on the travelway. The impact caused him to strike his head leaving a laceration. EMTs placed him on a backboard and transported by ambulance to Springfield Memorial ER. Treatment included staples to close the wound. A CT Scan revealed transverse process fractures. He was admitted for observation and discharged 2/24/2010.
While operating the scraper, hit a hole or bump in the road, causing him to bounce in the seat and causing a strong pain in his lower back. X-rays and a CT scan revealed a Vertebral Compression Fracture and he was directed to return to work and placed on desk work only restriction.
While recovering high line and lacing it onto a supply car his left foot was run over by the car pinching his big toe causing a fracture and leaving a laceration that required sutures to close. Treated at St. John's Hospital ER and released to unrestricted duty.

Drove himself to Hospital ER in Lincoln where a diagnosis was inconclusive & pain medication was prescribed. An additional follow up with Bansal Occupational in Springfield put him on a weight & position restriction & 2 weeks therapy for a lumbar strain.
While driving a water car and looking to see if the sprays were working he turned forward and had driven into a splice box striking his left arm, face and chest. Transported by ambulance to St. John's ER where initial diagnosis revealed a cracked sternum, fractured ribs and a fractured left wrist. Admitted for observation expecting to be released 1/12/2009.
While travelling on a golf cart he grabbed a Heitzman steel beam to stretch himself, the beam fell knocking him off the cart, lacerating his right hand and straining his upper body. Transported by ambulance to Memorial ER where CAT-scan and X-rays revealed no fractures and the laceration was closed by suturing. He was directed to follow up with his personal physician.
While standing next to the refuse bin a chunk of refuse bounced off the scraper and struck him on the left ankle. Transported by private vehicle to St. John's ER and released with a `weight bearing as tolerable' restriction. He began missing work 10/8/2008 due to pain. A follow up visit to Midwest Occupational Health Associates (MOHA) is scheduled for 10/15/2008.
Employee was passenger in mantrip that ran over a 2" x 2" wooden block causing it to flip up and strike him on lower right leg. He was taken to hospital where x-ray revealed no fractures but because of severe swelling and bruising doctor has advised not to return to work until swelling subsides.
As ee was descending northwest haul road of impoundment a left front tire on the cat 631G scraper blew out, causing him to loose control and roll scraper over berm. He was treated at hospital for abrasions and contusions & released to work with no restriction. He has continued to have swelling from a large contusion on left thigh and to do surgery relieve pooled blood.
DRIVING GETMAN LRD220 TRACTOR DOWN MAIN SOUTH TRAVELWAY HAULING SUPPLIES INTO THE MINE. RON OVER A ROCK IN THE TRAVELWAY HITTING MY HEAD ON THE CANOPY. AT THE TIME OF ACCIDENT HE DID NOT MISS ANY WORK. AFTER HITTING HIS HEAD UNDERGROUND AFTER THE ACCIDENT AND BEING IN PAIN HE DECIDED TO SEE DOCTOR RESULTING IN TESTING REVEALING A HERNIA TO HIS NECK
A contractor was injured when his right arm was caught between roller and belt. He was in process of cleaning north east side of plant refuse belt when he became entangled. Accident resulted in fracture and laceration to right forearm.
