Employee was in #6 entry went to follow center dump across to put scoop on charge. The scoop became stuck in the rib when making turn and EE said side dump shuttle car traveling up #6 struck the operators compartment of the scoop hitting employee in left shoulder.

Driver lost control of loaded coal truck going down hill at the 9 post causing truck to wreck at the bottom at the 8 post. Driver went to the hospital and received stitches to left ear. Returned to work next day.

Truck driver was traveling Ball Hooter Hill when they saw 2 vehicles that were pulled over to side of road. Went to gear down, got truck out of gear, tried to put back in gear but didn't know what gear it was. Tried to use brakes but they didn't work. Decided to put truck in the ditch line to stop it. Truck turned over on its passenger side.

Employee was helping lap belt, they had pulled one pull of 52 feet. They started to pull the second pull and the belt was frozen and when it broke loose the stored energy resulted in the belt striking the employee in the left knee.

While riding out on Damascus ride #352, employee had their foot on top of the fire extinguisher guard and as the ride went through the dip on 11 belt, 12 1/2 block, the employee's left foot struck a roof bolt in the top causing leg to be bent back towards the employee. The preliminary results are at this time possible ligament damage. We are currently waiting on MRI results.

The bottom was laying sideways. The mantrip went into a dip. When the mantrip came out of dip, spare tire caught cable bolt tearing spare tire loose and it rolled over top of employee striking leg, chest, and face.

EE was operating a Fairchild Scoop. While entering the #7 entry LOC on 1 Section the EE used left hand to move the line curtain away from the scoop deck. EE's foot then slipped off of the brake pedal and the scoop drifted forward toward the face catching EE's left hand between the scoop deck and coal rib.

The employee was riding on a mantrip when the mantrip hit a supply trailer parked in the crosscut. The employee suffered a contusion to his right leg. The employee followed up with a physician on 2/10/2015 and was put off work.

Employee was riding on a 10-man rubber tire mantrip when the mantrip lost its brakes and drove through a set of equipment doors. The employee raised his feet to keep the door from falling on him and his right foot was struck by the top of the door, breaking bones in his foot.

While operating a Joy 21 S.C., the injured had his right hand resting on the bottom of the operator deck & he caught his rt. middle finger between the deck & the pavement.

This individual was operating a 488 S&S scoop. He had pulled the rubber-tired supply car into a crosscut. The chain was hooked to the bucket of the scoop and frame of the car. He raised up the bucket to get slack in the chain to unhook it. This caused the car to drop and smash his right thumb between supply car and bucket.

The injured employee was operating a mobile bridge unit at the time the incident occurred. The canopy of the bridge unit caught a roof bolt and stopped suddenly, jolting the employee. (Employee notified foreman on 7/21/2005 of injury.)
