EE was assisting a driver in locating a popping noise they heard. EE told driver to raise & lower the bed, as EE bent over to look & listen EE inadvertently placed fingers on the lip at the bottom of the hoist cylinder as it was being lowered. EE's fingers were caught between cylinder and lip causing avulsions to each fingers from tip to nail bed. Soft tissue damage only to tips.

Showing all 6EE driving on site, fell asleep, hit guard rail, drove straight to Gate. Another EE was working gate. Area mgr & Ops. mgr. contacted. Area mgr. went on site. Ambulance called to check EE. EE did not want to go to hospital, changed mind, and decided to go; complaining of stomach pain. EE went hospital for evaluation. EE released with no restrictions on 10/28/2018.
EE was operating a 993K loader loading binder. A piece of binder broke loose. The operator felt a tweek in lower back. EE did not receive medical treatment until 12/07/2016, the first treatment. EE was off work at that time with Meniers disease. Management thought that EE was still of work for the Meniers disease and not for the back injury up until 02/17/2017.
Employee had exited the mantrip at the end of EE's shift and was in walking across the parking lot when another employee, in their personal vehicle struck EE, knocking EE to the ground. Employee was able to get up with assistance.
Employee was operating a stand-up battery powered forklift in the warehouse. While backing up, employee steered the forklift in the wrong direction causing it to strike a steel pole. Employees left foot was caught between the forklift and the pole smashing the distal end of his foot from the side resulting in fractures.
Employee was operating and descending a loaded tractor and trailer on the Laxare haul road when the driver lost control at some point causing the tractor and trailer to overturn onto its left side in the ditch. Driver was extricated by mine personnel and transported to the ER via EMS.
