Employee failed to maintain control of a Case 440 skid steer while backing down the incline of a clean-up pile, causing the skid steer to tip over. While the employee initially stated EE was fine, several hours later EE complained of a headache and soreness and was taken for medical evaluation.

Employee was standing 15 feet away from a crusher counterweight while a hook and crane were being used to manually rotate the counterweight. The hook snapped and a piece of the hook flew approx. 20 feet down and at an outward angle, striking the employee in the left forearm. Result was a laceration requiring 3 stitches.
