Showing all 6EE was assisting with the installation of a pulley on the surface refuse belt. The pulley became bound up and a telehandler was being used to maneuver the pulley as to align the bolt holes. The pulley broke loose from the bolt that had been installed and shifted, striking the EE in the face.
