Showing all 4While operating the Caterpillar 988H loader joystick shifter and engaging the gear selection buttons the employee sustained a strained tendon at the base of left thumb in palm area.
After installing new forks, EE used forklift to facilitate loading of damaged forks into service truck. As EE slid and shimmed the second fork into the truck, a fork on the forklift shifted, allowing the heel of the damaged fork to drop in between the forks on the forklift. As it dropped, a bur on the damaged fork caught sleeve of EE pulling arm into the crush point between forks.
EE falls out of their personal vehicle fracturing left wrist.
