Showing all 22Ee's haul truck was being loaded. A large boulder was placed in the bed of the truck, causing the ee to be jostled in the cab. EE reported to EE's supervisor, was seen by the EMTs and refused treatment. On February 18 EE sought treatment with a chiropractor and filed a WC claim. Investigation completed and claim was accepted 03/28/19. Reporting for medical treatment.
Truck driver was jarred while being loaded and reported a back strain.
Empl. was being loaded when a rock dropped shaking the truck. Next day at 9:00 am empl. complained of neck pain. Empl. was evaluated by Physician & released to work with cautions to avoid "jarring and neck motion". Empl. chose not to return to work at this mine and has been permanently transferred to another non-mining job.
2 Labors and skid steer operator were installing French drain around portal sets. Labor was holding fabric told the operator to back up while that was happening labor started to stand up while operator was tilting bucket back and pick hand between bucket and wall.

Ee driving down wet ramp, truck slid, ee tried to slow truck by rubbing it against right rib. Truck hit bump, bounced to left rib, and ee struck left hip against cab. He was seen at the hospital, diagnosed with a contusion, and put off work pending follow-up.
Employee was a passenger on a crew bus that hit a large pothole that jarred the employee around in the seat. She was seen initially on 01/19/14, diagnosed with a strain and released to full duty. She was seen again on 02/12/14 and give a prescription for physical therapy, making this reportable for treatment.
