They decided to self treat and returned on 9/28/2022 requesting medical treatment. The employee was sent to the clinic at that time and received work restrictions and prescription medication.

On April 21, 2016 Employee reported pain on left elbow. EE was brought into the clinic where EE told to do stretches every hour for 5 minutes. EE was also advised to do physical therapy 3 times a week for two weeks.

08/21/2014 Employee was operating a haul truck. Employee felt lower back pain 08/22/2014. He reported his injury 08/25/2014 and was seen by a physician.

On 5/16/2013 employee ran over large rock with Cat Scraper which jerked the cab. He reported that his back was sore but this was not work related and the pain was from an ongoing back problem. On 5/28/2013 he reported he now thinks this is work related. He was sent for medical treatment and was prescribed medication and given lifting restrictions.
