Employee was cleaning scales with a skid steer when EE hit the gap in the panels with the bucket causing the machine to stop suddenly, jarring the employee forward causing EE's head to hit the cab.

Showing all 51 Driver and 5 Passengers were riding in a UTV during non-work related activities. The driver lost control of the UTV and the vehicle ended up on its side. The bump to the head is believed to happen at some point during this time.
EE was operating a walk-behind skid steer under a conveyor. EE pinched left hand between the bottom of the conveyor frame & the control panel of the skid steer.
Employee ran over rock with front end loader, causing minor back pain. He was taken to Heart of Texas Hospital in Brady, Texas, and was put on restricted duty until he has follow up on 11/6/2015. We have accommodated his restrictions and is currently working under those restrictions.
Associate was backing up a 770 CAT haul truck to stockpile. While backing up she was run into by a 980 CAT loader. She complained of pain and was taken to company doctor for evaluation. She was released back to work without restrictions, but refused and seeked further evaluation on her own. She was released back to work with restrictions. Case is currently under investigation
