Employee was operating a loaded end dump truck, while EE was steering with EE's left arm EE received a sharp pain in EE's left shoulder. The employee did not report EE's injury to a supervisor until 2 hours after EE's shift ended at 4:00 p.m. EE sought medical attention hours later and was informed by that doctor to take the rest of the week off and got referred to an ortho dr.

05/09/25 emp tipped a scraper onto its side while working on a slope. EE stated EE was not injured. On next scheduled shift 05/12, emp stated that EE injured right shoulder. Emp went to ER, X-rays done, was diagnosed with right shoulder pain, and released to return to work with no restrictions. The emp worked until 05/27 when MRI indicated EE could not work without restrictions.

An employee was driving a OB truck when EE hit a soft spot on the road. The employee reported to EE's supervisor at 9:45 a.m. that EE was experiencing back pain and wanted to seek medical attention. Employee was taken to a clinic for physician observation and was referred to their clinic's chiropractor.

An employee was operating a 20,000 gallon CAT 777D water truck when the truck lost traction on a watered road. When the truck reached a dry section of the road it caused the truck to tip on its side. The operator was wearing a seat belt and was transported to the hospital where ee received 3 stitches for a laceration on right arm.

Traveling to park loader at end of shift. Hit a rut in the haul road and jarred him in the seat. He did not feel any pain at that time. By mid-morning the next day he was feeling pain in his back and sought medical treatment.
