A supply trip left the 60# spur at Sugar Run, and was moving towards the slope. The injured was moving mantrip to switch out with two other motors, when the supply cars came uncoupled and struck the injured's mantrip. The employee did not sustain any broken bones, but is off work to see a neck specialist.

EE was unspooling 2" plastic slick line. The scoop started pulling the line to stretch it out, the end whipped off the spool and struck the EE in left leg. The EE worked until 2/20/20 until EE went back to the ER and was given off until 2/24/20.

IE went to check on the hyd.functions of a belt clamp for the slope belt. Noticed a lump of coal at the clamping part and went to push it away when the clamps set. EE got the tip of middle finger pinched. Cause of this injury was safety awareness of this employee. Compliance with rules/regs,PPE,mining systems not a factor, miner proficiency may be a factor.

The Employee was at the end of the rail on the 1 West Tailgate section. EE was in the process of tramming the scoop down the track entry, outby 2blks to load supplies. The scoop tires slid off the rail causing the scoop to slam down on the rail, and strain EE's neck. The employee was complying with all laws, company policies, and was wearing all appropriate PPE.

The employee was sitting inside the cab of a company motor 4B when another motor and dust pod car became uncoupled rolling back and striking employee's motor. When the cars collided an object flew off one of the cars striking the employee on the right side of head causing a laceration and a concussion. The employee was wearing all the appropriate PPE.

The employee was traveling on 9 South Mainline Haulage in #70 7man diesel mantrip. The mantrip struck a broken rail at 146block 9 south haulage, causing the employee to strike multiple parts of body against the mantrip, neck, back, knee, and shoulders. The employee didn't violate any laws or company polices, and was properly trained.

The employee was riding in a mantrip along 9 South Haulage. The mantrip picked 16D switch, went into the spur and struck a parked ride before stopping. The employee was thrown back against the seat and struck neck area on the SCSR storage box. The employee didn't violate and law or company polices, and was wearing all the appropriate PPE.

The ee was operating a CAT 740B Articulating truck when the bottom center pin sheared off causing the cab of the truck to tilt forward jolting the operator. The ee stated ee had pain in lower back and left leg numbness. The ee did not violate any laws or company policies and was wearing seat belt. The ee pre-op-ed equipment prior to operation.

The employee was tramming a scoop in 7 East Section in the #2 heading. The employee was resting EE's hand partially outside of the operator's compartment and was tramming close to the rib on the operator's side. The employee smashed EE's left hand between scoop and the door.

The employee was cleaning outby the temporary tailpiece on #4 belt, when a rock came off the belt and struck them in the leg. The employee did not violate and laws or company policies and was wearing all the appropriate PPE.

While the EE was riding on a mantrip at the end of shift, the mantrip the EE was riding on slid into another mantrip causing the EE to be thrown into the back of the seat hitting the EE's back and ribs. The EE was wearing all the required PPE.

Injured was passenger in D38 mantrip when it pulled up to East Mains Switch and collided with another mantrip trying to get on the back side of East Mains Switch. The injured braced for impact and felt pain in lower back, being diagnosed with a muscle strain of the lumbar region.

Moving power in 5E #2 entry near 10 blk with scoop pulling power train (power car, high voltage tub, then E-sled). Injured went to hang high voltage cable when the tire of the E-sled rolled onto left foot, resulting in contusions to foot and a laceration on left shin that required 4 sutures to close.

Injured was pulling supplies with a Brookeville diesel locomotive. Injured stood up, looked inby to check trip, struck the back of head against a steel-set beam and then struck lip against the locomotive windshield. This resulted in a laceration to lip, requiring 20 sutures to close.

Driving D87 mantrip down 9 South Haulage and went into 21D switch that was left thrown for the turn, striking a parked mantrip and receiving contusions when knee and lower leg struck components in the operators compartment.

Turning around 620 scoop in 2-1 crosscut at 30 block in 1E Headgate and drug operator compartment across rib until a rib strap hit the scoop door and caused the door/strap to strike EE in the left shoulder blade, resulting in a contusion. No problem with PPE, training, mining equipment or compliance issues.

Employee was traveling down the 9 South haulage on a diesel locomotive. Leaned down to turn inby headlights off and when sat back up hit head on the mine roof. No problems with mining equipment, ppe, training or compliance issues. Employee started missing work on 01/04/2017.

Moving power center outby in 1E track entry, with power center on T2 trucks, and hooked power center to stationary 620 scoop bucket (up #2) while tramming T2 trucks to decrease turn radius from crosscut to go outby. Injured was operating T2 truck controls from umbilical cord, in the 1-2 crosscut, when the chain broke and hit the employee in knees.

Removing handrail from panline in 23W with 650 scoop. Bill of scoop put against panline frame, winch hook attached to center of handrail, injured stood by front scoop tire. Handrail caught, built tension in winch, and then released, throwing handrail on injured, breaking two fingers and bruising thigh. No problems with PPE, training, mining methods or compliance issues.

Injured was guiding belt onto a flat car in the 2 East belt spur as it came off the belt winder, when the loose belt was dropped and struck employee in the head, knocking employee down. Employee was treated for contusions and soft tissue damage to the neck and upper back. No problems with mining methods, PPE, training or compliance issues.

Injured riding on inby side of 15 man diesel mantrip. Mantrip operator did not notice that 206 spur (9 South) was thrown for turn, or motors were present and ran into the switch, striking a motor. Injured struck head and left knee on interior of mantrip. No problem with mining methods, training, PPE, or compliance issues.
