At intersection of DLS, Fuel truck driver saw a truck coming, fuel driver turned sharp to avoid truck and the fuel truck tipped over. Driver exited the truck. Driver noticed fuel leaking from the top of the truck. Driver climbed back inside the cab, turn off the engine and lights to the fuel truck. A blade put up a berm to contain the spill. Driver was taken to the EMT and treated.

Showing all 3Fueler was driving fuel truck and drover over a windrow. This caused the steering wheel to jerk and pull EE's left arm. Fueler reported the incident and the EMT looked at EE's arm. EE returned to work. On 8/8/17 the employee informed the mine that EE has been taken off work and will be receiving surgery due to injuries from the incident.
Water truck driver reported a noise while steering to the right. Pit mechanic installed new grease lines, the mechanic instructed the water truck driver to steer the wheels from side to side as grease was added. As the wheels were turned to the right the mechanic was pinned between the tire and frame causing injuries to the mechanic, 4 broken ribs and a broken collar bone.
