Showing all 5Employee was removing bolts to install skirt on the DLA hopper when the wrench employee was using was caught by the conveyor and pinched the right index finger between the wrench and a hopper bracket.
Crew was swapping out a 12" pump at #6 pond. While moving pump to new location, with an extended boom forklift, pump was suspended from the forklift by rigging. A miner was walking along side of the pump. The pump was about 6" above the ground when it hit a high spot which pushed the pump against the forklift outrigger, smashing the end digit small finger left hand.
Employee was using forklift to transport 55 gal oil drum to powerhouse area. In the process of moving drum from warehouse pallet to transport pallet, drum rolled and pinched/smashed right middle and ring finger causing fractures to top of both digits.
Employee was driving a cart, became distracted and drove into a hole striking a pipe. Employee was taken for medical care at Gulf Coast and (R) elbow x-ray was negative. First visit to Dr. on 2-6-2019 and EE was returned to full duty. 2nd visit was on 2-20-2019 and MRI was ordered and EE was placed on 0-10 lbs. upper extremity restrictions.
