Using FCT to pull cable slack with loops of cable tied to a drill steel placed on the FCT while in the 8 West belt entry. Injured was holding drill steel to keep the rope from pulling off, but the rope pulled off anyways and the drill steel came back and struck them in upper left leg resulting in a contusion. No problems with equipment or compliance issues, training or PPE.

Employee pulling a power car in 7 West 3 left 3 block intersection by hooking a chain up between the power car and a scoop. They had scoop back up to take slack out of the chain and it failed immediately, throwing a link that struck them in the forehead resulting in 3 lacerations that required 6 sutures total to close. No problems with equipment issues, training or PPE.

While applying a chain around a recently coupled supply car to a motor when the motor on the other side of the trip tried to uncouple, but the couplers got stuck and ended up pulling the supply cars and motor a short distance causing a strain to their shoulder and upper back. Started missing work 9/20/25. No problems with equipment or compliance issues, training or PPE.

Injured operating 15 man personnel carrier down 9 South haulage between 90 and 120 block. Foot came off deadman switch, causing vehicle to go into neutral, injured looked down to switch vehicle back into direction and struck EE's hard hat off the mine roof. No problems with equipment or compliance issues, training or PPE.

While trying to uncouple an empty flat car that was off track from motor, they had the other motor pull on the car but it did not uncouple and shifted causing the coupler to swing and pinch left calf between the coupler and the motor resulting in a contusion. 64 block 9 north track. No problem with equipment issues, training or PPE.

Injured was hooking a chain onto a roll of belt inby 10 belt take up to be moved by the belt winder when another employee accidentally hit the lever of the belt winder causing the chain to tighten and catch the injured right pinky between the chain resulting in a distal tuft fracture. No problems with equipment, compliance issues, training or PPE.

While pulling fuel buffalo with 24 ton diesel motor through 118 1/4 block 9 south haulage they heard a noise like the buffalo went off track. Without stopping they turned back to look at the buffalo and struck their hard hat off a roof strap, resulting in an injury/aggravation to cervical spine. No problems with equipment or compliance issues, training or PPE

While operating golf cart going into 60# off 1 North haulage the injured struck hard hat off a wooden roof plank, causing pain in neck, upper back and shoulders. No problems with equipment or compliance issues, training or PPE.

IE lifted the boom of a microtraxx machine. While IE reached in with right arm to check the boom pin, IE leaned on the remote box with chest, engaged the boom down function and lowered the boom onto IE's right arm. IE was trained as a microtraxx operator, but this was a compliance issue, as the machine was not de-energized, locked, tagged, or blocked against motion.

Injured passenger in mantrip that collided with parked diesel dusters at 47 block St Leo. Some material fell out of the top and got into the mantrip operators eyes and when EE got their vision clear, EE was too close to dusters to stop in time. Injured broke right knee during collision. No problems with equipment or compliance issues, training or PPE.

Driving #9 Shuttle Car around turn at 98 blk #6 and kept turning into 5-6 crosscut, striking operator compartment against rib and dislodging a rib bolt strap into operator compartment striking EE's upper back and causing a contusion. No problems with equipment or compliance issues, training or PPE.

Driving mantrip down St Leo haulage when EE encountered a rope hanger at 40 blk St Leo Haulage that had an S hook hanging on it which caught the hood of EE's sweatshirt tearing the hood and causing pain in EE's neck area. No problems with equipment or compliance issues, training or PPE. The rope hanger has been removed.

Injured was driving a shuttle car in the 7 West outby left ladder and ran over a rock in the tram road which caused injured to strike hard hat off the canopy which resulted in a strain/sprain to neck. No problems with equipment or compliance issues, training or PPE.

Employee was passenger in a mantrip exiting the mine when they encountered a track switch left thrown for the turn. The caused the mantrip to enter the spur and strike a parked (vacant) mantrip. The impact resulted in the employee sustaining a sprained left ankle.

The EE was setting a post on the tight side of St. Leo Haulage at 12 blk. An approaching ride thought the injured was in clear. The mantrip coasted past the area, and the injured was contacted by the mantrip. The injured finished the shift, but began missing 2/21/24.

The injured EE was operating the tail motor of a supply trip heading back to Sugar Run. The EE had head down in a lower, and did not hear partner telling EE they were going to stop. The injured couldn't stop in time, and contacted an empty flat. The EE sustained a chipped tooth.

Operating 636 scoop to flip a belt sled over on the 3 West longwall section when the sled went under the scoop bucket and the scoop tires drove over the belt sled. When brakes applied the scoop came down off the belt sled and jarred the driver causing pain in lower back and neck. No problems with equipment or compliance issues, training or PPE.

EE was riding in a mantrip along St. Leo Haulage. The mantrip came off track, and the EE's head struck the inside of the mantrip. EE began missing work on 12/29/22.

The injured was riding on the inby end of a mantrip traveling towards Miracle Run Bottom on St. Leo Haulage. The mantrip changed directions and started traveling back inby. The mantrip contacted a motor around 42 block St. Leo, causing the injured to hit their head in the mantrip. EE missed the next two scheduled shifts, and returned to work on 6/20/22.

EE was running a motor on 1 North Haulage, and ducked to go under a low spot on the haulage. When EE raised up, a rock dust hose caught EE's hard hat. This intern strained the EE's neck. EE began missing 8/19/21.

The EE was walking along the FCT towards the DMU. The tail roller failed in the DMU, which created some light smoke. The injured inhaled some smoke before EE was able to leave the belt entry. EE was brought outside and transported to a medical facility for further evaluation. No lasting damage was discovered, but EE was given two days off.

Employee was operating #2 trolley jeep. Employee forgot safety glasses and went back outside to retrieve them upon returning employee got back into jeep and pumped the brake and starting tramming forward. Employee looked down to turn on lights, while doing so crashed into #61 trolley jeep. Employee injured left leg. Employee did not start missing work until 4/4/2021.

The injured EE was operating the tail motor of a supply trip. They were putting two cars away in 160 spur on 9 South Haulage. The lead motor was pulled inby the spur with one car, as person exited motor, the motor and car was struck by the tail motorman and one car. The tail motorman struck face off of the firewall/control area of the operator's compartment.

The EE was running motor on 9 South Haulage. Rock dust/coal, fell from the mine roof and got into the EE's right eye. EE finished EE's shift and went to the doctor on 11/4/20. EE received prescription eye drops.
