EMPLOYEE WAS NOT WEARING A SEAT BELT, HIS MOMENTUM BROUGHT HIM FORWARD AND HE STRUCK HIS RIGHT LEG AGAINST THE STEERING WHEEL RESULTING
EMPLOYEE WAS INJURED ON 6-20-88 WHEN A TROLLEY CAME OFF THE MONO-RAIL, STRUCK AND BROKE HALF THE BILL OFF HIS HARD HAT RESULTING IN LACERATIONS TO FACE AND CERVICAL STRAIN. A A RESULT OF CONTI NUED NECK PAIN EMPLOYEE WAS SCHEDULED FOR A MEDICAL TEST ON 9-13-88 AND WAS ABSENT FROM WORK ON THAT DAY.

EMPLOYEE WAS LEAVING #14 SHOVEL WITH A LOAD OF ROCK ON PRODUCTION TRUCK 5050. HE HIT A POT HOLE IN THE ROAD AND JARRED HIS NECK.
HE WAS DRIVING TRUCK ON HIS WAY TO SHOVEL AND HIT A HOLE OR BUMP WHICH CAUSED HIM TO HIT HIS HEAD INTO THE ROOF OF THE TRUCK AND IT RESULTED IN PAIN INTO HIS NECK AND DOWN INTO HIS BACK.
THIS EMPLOYEE SUFFERED A RECURRENCE OF OLD INJURY WHICH OCCURRED ON 4-14-85 NO NEW WORK RELATED ACTIVITY IDENTIFIED.
INJURED WAS GOING TO CLEAN OFF THE RAILS ON TRACK #7. DUE TO A DEFECTIVE RAIL, THE LOADER BUCKET CAUGHT THIS DEFECT AND CAME TO A SUDDEN STOP. THIS CAUSED THE INJURED TO BANG HIS FOREHEAD ON THE WINDSHIELD, BREAKING THE BRIM OF HIS HARDHAT, CRACKING THE WINDSHIELD. HE RECEIVED A SLIGHT CUT ON HIS FOREHEAD, A CONCUSSION, CERVICAL STRAIN, SORE KNEE.
INJURED WAS STANDING ON THE WORK PLATFORM BY THE FEED WINDOW OF #16 PRIM MILL, HIS FOOT POSITIONED UNDER THE SPLASH RING. THE FORKLIFT OPERATOR WAS MOVING A SCRAP BOX INTO POSITION AND STRUCK THE PLATFORM, CAUSING IT TO TILT UP, JAMMIMG INJURED'S FOOT BETWEEN THE SPLASH RING AND THE PLATFORM, FRACTURING HIS LEFT GREAT TOE.
EMPLOYEE WAS HELPING SEVERAL OTHER EMPLOYEES EMPTY COAL CARS.THE CONTROLS FOR THE CAR SHAKER WERE TIED OVE AN ANGLE IRON ON THE WALL WITH A ROPE.THE ROPE VIBRATED OFF AS THE SHAKER MOVED ALONG THE CAR AND THE CONTROL BOX STRUCK EMPLOYEE RERSULTING IN A LACERATION/RIGHT EAR REQUIRING SUTURES.

A CHUNK GOT CAUGHT IN THE WHEEL CAUSING THE WHEEL TO JAM TO WHICH CAUSE8 THE CABLE TO TIGHTEN. EMPLOYEE WAS TRYING TO STOP THE CABLE FROM COMING OUT OF THE DRAY BY PUSHING HIS FOOT ON IT. WHEN THE CABLE TIGHTENED IT HIT EMPLOYEE ON THE LOWER LEFT L
