THE EE REACHED OVER TO PUSH ON THE HEADLIGHT SWITCH ON HIS HAUL TRUCK AND FEL A PAIN IN HIS WRIST.
EE MAY HAVE RUN OVER ROCK IN WORKPLACE SPRAINED BACK THE EE LATER WENT TO DR & SEVERAL THERAPY SESSIONS WERE SCHEDULED. NO LOST OR RESTRICTED DAYS

THE EE STATED THAT HE WAS DRIVING HIS HAUL TRUCK UP TO THE HEAP DUMP WHEN HE HIT A POTHOLE IN THE HAUL ROAD AND FELT A SHARP PAIN IN HIS LEFT TESTICLE.
THE EE REPORTED WHILE CROSSING A ROUGH SECTION OF TRACK WITH A LOCOMOTIVE ON THE 985 HAULAGE LEVEL HE TWISTED HIS NECK AND SHOULDER. A HERNIATEDCERVICAL DISC RESULTED. LOST WORKDAYS BEGAN ON 7/29/98
EE ATTEMPTED TO ADJUST SEAT HEIGHT WHILE EQUIPMENT WAS IN MOTION IRAN TRUCK INTO RIB CUT HAND WHEN SEAT CAME DOWN STITCHES.
WHILE ATTEMPTING TO UNPLUG 2 STANDARD FEED CHUTE, A ROCK CAME DOWN AND STRUCK EE ON THE TIP OF HIS RING FINGER. INITIAL DIAGNOSIS WAS A CONTUSION OF THE FINGER, BUT AFTER A COUPLE OF DAYS WAS SEEN BY A SPEICALIST AND THE DIAGNOSIS WAS A TURT FRACTURE.
EMPLOYEE CLAIMS SHOULDER STRAIN-HIT ROCK BACKINGTO MULL HOLE? EMPLOYE HAD REPORTABLE ACCIDENT 6.24.98. TO LEFT HAND STITCHES NO WORK AVAILABLE, SENT HOME 7.6.98. DR. TO EVALUATE 7.16.98.
THE EMPLOYEE WAS DRIVING A WATER TRUCK AND HIT A BUMP IN THE HAUL ROAD AND HER LEFT KNEE HIT AGAINST THE CAB DOOR AND BRUISED HER KNEE.
WHILE RIDING IN THE BACK OF A TRACTOR, EMPLOYEE COMPLAINED OF A WRIST INJURY AND LOWER BACK PAIN.
WHILE CLEANING BALL CHIPS OFF CONVEYOR MAGNET CHUTE, A REJECT BALL FELL FROM CHUTE STRIKING ANOTHER BALL AND A METAL CHIP STRUCK EE IN LEFT EYE.
EMPLOYEE WAS RIDING IN THE 2ND TO LAST CAR OF ANEMPTY TRAIN, WHEN THE TRAIN DERAILED CAUSING THEDOOR TO OPEN AND THE LEFT LEG THEN ENTERED THE PINCH-POINT. THERE WERE NO FRACTURES, THE LEG WAS BRUISED BADLY. THERE WERE SAFETY EQUIPMENT SAFEGUARDS PROVIDED BUT WERE NOT USED.
THE EMPLOYEE WAS RIDING IN A CREW BUS GOING TO HIS TRUCK WHEN THE BUS DRIVER HIT A POT HOLE IN THE ROAD AND THE EMPLOYEE WAS BOUNCED IN HIS SEAT.
WHILE BACKING AWAY FROM A DRILL IN A PICK-UP, EE DROVE OVER A LARGE ROCK CAUSING HIS HEAD TO PULL SIDEWAYS.
EMPLOYE EBACKED TRUCK INTO RIB, BACK STRAIN.
THE EMPLOYEE ALLEDGED THAT HE BEGAN TO EXPERIENCE PAIN IN HIS UPPER BACK WHILE DRIVING A 170 TON HAUL TRUCK. THE EMPLOYEE DID NOT REPORT THE INCIDENT UNTIL 11-10-97. NOTE: THE EMPLOYEE WAS GIV EN WORK RESTRICTIONS ON 11-14-97.
WHILE BACKING UP UNDER A SHOVEL BUCKET IN A HAULTRUCK, EE STRUCK THE SHOVEL BUCKET.
TRACTOR HIT RIB REPORTABLE DUE TO THERAPY NO RESTRICTED DUTY.
WHILE TRUCK 304 WAS BACKING INTO 516 SHOVEL, TRUCK 342, WAS MAKING A RIGHT TURN AND IN DOING SO, TRUCK 304 STRUCK TRUCK 342. DAMAGE WAS MINIMAL HOWEVER THE DRIVER OF 342 SUFFERED FROM WHIPLASH AND HEADACHES.
THE EE HAD COMPLTED REPAIRS ON A MAN LIFT ON THE#18 SHOVEL. AND AS HE WAS ATTEMPTING TO RELEASE THE MAN LIFT FROM ITS LOCKING PIN THE MAN LIFT DROPPED DOWN CAUSING HIM TO LOOSE BALANCE AND FALL TO THE GROUND.
EE STATES HE HIT HIS LEFT KNEE ON TRACTOR COMPARTMENT. BEGAN LOSING TIME 12/16/97.
AS 381 PICK UP TRUCK WAS BEING BACKED UP INTO THE SHOVEL REPAIR BAY, EE WAS STRUCK BY THE REAR OF THE TRUCK INJRING HIS LOWER BACK AND NECK.
WHILE CONDUCTING A PREOPERATIONAL CHECK OF HIS HAUL TRUCK, THE AIR HOSE DISCONNEDTED AND IN DOING SO HIT EE ON THE SIDE OF HIS KNEE & HIS LOWER BACK
EE SUFFERED BACK SPRAIN--TRACTOR STRUCK BY EQUIPMENT--EE WAS CHECKED BY DR. AND RECEIVED NO RESTRICTIONS--HE ATTENEDE SEVER THERAPY SESSIONS AND DID NOT DO HIS REGULAR JOB--TOTAL=3DAYS OF REST RICTED. THE INFORMATION WAS NO KNOWN DURING THE REQUIRED 10 DAY REPORTING REQUIREMENT.
WHILE OPERATING A HAUL TRUCK, EE HIT A BUMP IN THE ROAD AND IN THE PROCESS EXPERIENCED LOW BACK PAIN.
EE CLAIMS THAT OPERATING SCALER CONTROLS FOOT CONTROL CAUSED HIS KNEE TO SWELL. EE BEGAN LOSING TIME 11-15-96.
THE EMPLOYEE REPORTED DRIVING THE FUEL/OIL RIG BY A TRAIN TO BE SERVICED WHEN THE LEFT FRONT TIRE SLIPPED OFF OF THE TRACK ADN FELL IN A DEEP WATER HOLE. THE JJARRYING OF THE FUEL/OIL RIG CAU SED THE EMPLOYEE'S HEAD TO STRIKE AGAINST THE CANOPY FORCING HIS AW CLOSED. A BROKEN LOWER LEFT MOLAR RESULTED.
WHILE RIDING IN THE BACK OF A MAN TRUCK. EE SHUT THE DOOR ON HIS RIGHT RING FINGER. THIS RESULTED IN A TUFT FRACTURE OF THE FINGER WITH SUTURES NEEDED.
EE CLAIMS TO HIT KNEE ON OPERATOR COMPARTMENT WHEN HE APPLIED BRAKE.
THE EMPLOYEE HAD RAISED A TRASH DUMPSTER UP ON A FORK LIFT TO DUMP THE TRASH AS HE REACH DUP TO PUSH THE DUMP LEVER HE HAD HIS LEFT HAND RESTING UNDER THE DUMPSTER SUPPORT BRACKET AND WHEN THE DUMPSTER TIPPED FOREWARD THE WEIGHT SHIFTED AND HE CRUSHED THE FIRST JOINT OF HIS MIDDLE FINGER BETWEEN THE FOR AND BRACKET.
EMP WAS IN THE ENGINE COMPARTMENT OF A LOADER ASSISTING IN THE REMOVAL OF A STEERING CYLINDER. THE ENGINE COMPARTMENT DOOR HAD BEEN LEFT OPEN. AS THEY MOVED THE LOADER TO REMOVE THE CYLINDER, THE DOOR CLOSED AND HIT HIM ON THE ARM. SUFFEREDPARTIALLY DISLOCATED SHOULDER AND HAS BEEN OFF FROM WORK SINCE THE ACCIDENT.
THE EMPLOYEE REPORTED DRIVIGN A LETMAN LUBE TRUCK THROUGH A DEEP WATER HOLE. AN ACUTE LUMBER STRAIN RESULTED FROM THE JARRING OF THE LUBE TRUCK. THE EMPLOYEE DID NOT SEE A DR UNTIL 7-11-96. THE EMPLOYEE RETURNED TO WROK ON 7-11-96. RESTRICTE DDUTH. THREE WORKDAYS.
WHILE GOIN DOWN A RAMP @ APPROX 5-8 MPH, EE LOST HIS DYNAMC BRAKE AND IN DOING SO, STOPPED HIS TRUCK BY HITTING A BERM. THIS RESULTED IN PAIN TO EE'S NECK AND LOWER BACK.
EMPLOYEE WAS OPERATING A TRUCK. A WHEEL BROKE OFF AND HE HIT THE DASH. HE SUFFERED A SHOULDER BRUISE. HE BEGAN LOSING TIME ON 6-4-96.
EMPLOYEE REPORTED ENTERING THE #2 BELT LINE DECLINE WHICH STILL HAD ROCK DUST AND METAL RUST IN THE AIR AFTER THE BELT HAD BROKEN. A RUST PARTICLE BECAME EMBEDDED IN HIS RIGHT EYE. EMP WAS WEA RING SAFETY GLASSES.
THE EE WAS RIDING IN THE BED OF A PICKUP TRUCK THAT WAS ENROUTE TO THE CHANGEROOM AFTER THE B-SEHIFT. THE TRUCK HIT A BUMP, WHICH CAUSED THE EE TO GO AIRBORNE. EE LANDED AGAINST THE WHEEL WELL WITH HER TAILBONE AND LOWER BACK. DAMAGE TO COCCYGEAL AND RECTAL AREAS.
WHILE CHECKING IDLERS ON #136 CONVEYOR, EE SLIPPED ON THE DIAMOND PLATER WALKWAY CAUSING HIS LEFT ARM TO GET CAUGHT BETWEEN THE IDLER AND THE BELT. EE RECEIVED A BROKEN ARM IN THE PROCESS.
THE EE WAS OPERATING 475 HAUL TRUCK, HAULING FROM THE 15 SHOVEL WEST PIT TO THE CR-3 CRUSHER. THE EE STATED THAT AS HE WAS TRAVELING DOWN THE RAMP, HE LOST HIS DYNAMIC BRAKES AND WAS UNABLE TO STOP. THE TRUCK TRAVELED THUR A BERM AT THE BOTTOM OF THE RAMP AND TURNED OVER ON TO ITS RIGHT SIDE. THE ACCIDENT IS STILL UNDER INVESTIGATION HOWEVER THE TRUCKS SERVICE BRAKES WERE IN WORKIN
THE EMPLOYEE WAS STANDING UNDER #492 HAUL TRUCK , CHECKING FOR LEAKS, WHEN DIRT FELL INTO HIS EYE.
EE WAS TRYING TO RERAIL A CAR WHEN IT ROLLED AND PINCHED HIS TOE. HE SUFFERED A FRACTURED LITTLE TOE AND WAS PLACED ON RESTRICTED DUTY.
EMP WAS OPERATING 637 WHEN SEAT BELT CAME LOOSE (UNBUCKLED). BOUNCED AROUND IN CAB UNTIL EMP GOTMACHINE STOPPED TO REBUCKLE SEATBELT. RIGHT LOWER BACK WAS HURTING, THIS OCCURRENCE.
WHILE PULLING AWAY FROM AN INTERSECTION THE LUBE TRUCK HIT A BUMP & THE SEAT BOTTOMED OUT & EE EXPERIENCED LOW BACK PAIN
EMPLOYEE WAS DRIVING A TRUCK OVER A ROUGH ROAD AND HURT HIS BACK. HE HAS BEEN TREATED CONSERVATIVELY BUT HE WAS NTO PROGRESSING AS WELL AS EXPECTED. HE BEGAN LIGHT DUTY ON 7-24-95.
THE EMPLOYEE WAS ENROUTE TO WORK IN HIS PERSONAL CAR AND WAS HIT BY ANOTHER EMPLOYEE WHO WAS ON HIS WAY HOME AT THE INTERSECTION OF THE RAY MINE ROAD AND SX-EW ROAD.
EE WAS RIDING TO WORK WITH HIS FATHER AND THEY WERE HIT BY ANOTHER VEHICLE WHO WAS ON HIS WAY TOHOME AT THE RAY MINE ROAD AND SX-EW INTERSECTION.
EMPLOYEE WAS MUCKING ORE WITH A FRONT END LOADER A SMALL ROCK FELL BOUNCED OFF THE LOADER & HT HIS CHIN HE RECEIVED A LACERATION WHICH REQUIRED STITCHES
