EE WAS CLEANING IN THE SURGE TUNNEL USING A SHOVEL AND PUTTING THE MATERIAL ONTO THE BELT THE SHOVEL CAME INTO CONTACT WITH THE BELT CAUSING THE SHOVEL TO KICK BACK STRIKING HIM IN HIS RIGHT E YE. HE WAS WEARING HIS AFETY GLASSES BUT THE LENS BROKE AND GLASS GOT INTO HIS EYE. THERE WAS NO DAYS LOST.

EE WAS CHECKING A NOISE ON THE DOUBLE DECK SCREEN OF THE WASH PLANT WHEN HE INSERTED HIS LEFT HAND INTO THE DISCHARGE END OF THE SCREEN. THE LEFT TIP OF HIS INDEX FINGER CAME IN CONTACT WITH T HE 3/8" DISCHARGE PLATE AND IT CAUGHT THE TIP OF HIS LEF TINDEX FINGER. IT BROKE AND CUT THE FINGER. ALL GUARDING AND SAFEGUARDS WERE IN PLACE.
EE WAS OPERATING A HAUL BEING LOADED WITH ROCK. THE LOADER DUMPED FIRST BUCKET OF MUCK IN TRUCK JERKING THE TRUCK CAUSING BACK PAIN. ON 8/26/98 EE DROVE HAUL OVER ROUGH ROAD WAY WHICH TWISTED HIS BACK AGAIN CAUSING LOST TIME BEGINNING 8/27/98.
EMPLOYEE WAS OPERATING A WATER TRUCK. HE WAS GOING UP A INCLINE. HE STOPPED TO FILL UP THE WATER TANK ON THE ROOF BOLTER. HE SAID HE APPLIED THE PARK BRAKE AND WAS GOING TO GET OFF TO FILL THE WATER TANK ON THE BOLTER HE SAID IT STARTED SLIDING BACK OR ROLLING BACK AND BUMPED INTO THE BOLTER. HE SAID HE HURT HIS ARM AND BACK.
EMPLOYEE WAS BACKING IN A FACE AREA TO GET HIS TRUCK LOADED WITH MUCK. HE BACKED INTO A LEDGE AND THIS JARRED HIM AROUND AND STARTED HIS NECK ADNB SHOULDER CAUSING HIM PAIN.
EE WAS HAULING BLASTED ROCK TO THE CRUSHER WITH A CAT 631D HAUL TRUCK, WHEN AS HE WENT THRU A CROSSCUT THE RIGHT FRONT TIRE BLEW OUT CAUSING THE TRUCK TO HIT THE RIB ON THE RIGHT SIDE. HE SUBS TAINED 2 SMALL CUTS ON HIS HEAD & ONE REQUIRED 2 STITCHES.
EE SAID WHILE OPERATING A 988 CAT LOADER THE JERKING AND BOUNCING AROUND CAUSED HE NECK TO STARTED HER NECK TO HURT.
EMP WAS TRYING TO GET THE ACCUMULATOR BELT IN THE BAGHOUSE TO MOVE. THE CONVEYOR BELT WAS SLACK. LUBRICATED SOME OF THE ROLLERS AND BELT STARTED TO MOVE BECAUSE HE HAD NOT TURNED OFF POWER. TH IS BELT DRIVES ROLLERS THAT MOVE BAGGED LIME. A ROLLER CAUGHT HIS FINGER CUTTING IT.
EE WAS RIDING ONT HE BACK OF A TRACTO9R GOING TOWORK AREA. THE TRACTOR RUN OVER A ROUGH PLACE IN THE FLOOR AND IT JARED HIM AND HE SAID IT HURT HIS BACK
EMP WAS GUIDING A FLIGHT BAR INTO POSITION AT STAMLER CRUSHER. HAD HIS RIGHT HAND ON MAINTENANCETRUCK CABLE JUST ABOVE SHEAVE BLOCK AND TOLD TRUCK OPERATOR TO RAISE THE BAR. WHEN CABLE WINCHED UP, IT CAUGHT EMPS FINGER BETWEEN SHEAVE PULLEY AND CABLE. FINGER REQUIRED 5 STITCHES TO CLOSE WOUND.

INJURED AND ANOTHER EMPLOYEE WWERE HELPING TO GET ONE RAIL CAR OF TWO THAT WERE CONNECTED READY FOR THE FREIGHT TRAIN TO PICKUP. EMPLOYEE HAD SET BRAKES ON ONE CAR AND WAS BETWEEN THE TWO CARS RELEASING THE COUPLING. THE CAR MOVED AWAY AND THE EMPLOYEE STEPPED BETWEEN THE COUPLINGS OF THE 2 CARS. THE TRAIN BUMPED THE CAR TO CONNECT & THE CAR MOVED BACK TO ORIGINAL POSITION PINCHING
EMPLOYEE WAS RIDING THE MANTRIP TO GO UNDERGROUND. THE CAR WAS ABOUT 200 FT FROM THE BOTTOM. THE CAR HIT A ROCK AND DERAILED. EMPLOYEE STOOD UP IN MANTRIP CAR WHEN THE FRONT CAR HIT THE BACK C AR IT THREW HIM BACK INTO HIS SEAT HURTING HIS BACK. LAST DAY WORKED 2-8-96.
EMPLOYEE WAS RIDING MANTRIP CAR UNDERGROUND. THE CAR JUMPED THE TRACK AND HE SAID IT JERKED HIM AROUND SOME AND THE D-RING ON HIS MINER'S BELT CAUSED HIM TO HURT HIS LOWER BACK.

EMPLOYEE WAS MUCKING OUT A FACE WITH 988B CAT LOADER. HE SAID THAT THE BUCKE TSTRUCK A LEDGE, CAUSING HIM TO GO FORWARD AND HHIS BACK STARTED TO HURT.
EMPOYEE WAS ON THE MANTRIP GOING UNDERGROUND. T THE MANTRIP CAR JUMMPED THE TRACK. ADN WAS STRUCK BY THE FRONT CAR, CAUSING A SUDDEN MOVEMENT. EMPLOYEE SAID THA TCAUSED HIS BACK TO START HUR TING.
EMPLOYEE WAS PUSHING RAIL CARS WITH A 850 CAT. LOADER. HE COULDN'T SEE AND PUSHED RAIL CARS INTO PARKED CARS. THIS THREW OPERATOR FORWARD CAUSING HIM TO JAM HIS NECK. OPERATOR HAD SPRAIN IN TH E BACK OF HIS NECK. THE LOADER OPERATOR AND THE BULK LOADER HAS TWO WAY RADIOS TO PREVENT THIS TYPE OF ACCIDENT FROM OCCURRING.
EMPLOYEE WAS MUCKING OUT IN 12 WEST WITH A 988 CAT LOADER. HE NOTICED SOME ROCK FALLING FROM ROOF. HE PUT LOADER INTO REVERSE & BACKED OUT FAST. HE SAID THE LOADER JERKED HIM AROUND & HURT H IS BACK & NECK. FIRST DAY MISSED WAS 3/11/94.
EMPLOYEE OPERATING LOADED HAUL TRUCK COMING UP RAMP FROM LEVEL FOUR. THE TRUCK WAS NEAR CREST OF RAMP WHEN THE RIGHT AXLE BROKE. OPERATOR BACK TRUCKAPPROX 50' THENTURNED TRUCK SHARP TO A ROA D ONTHE LEFT. WHENT HE TRUCK TURNED THE FRONT SLID AROUND AND THE TRUCK ROLLE DOVER ON ITS RIGHTSIDE. THE TOPERATOR HAD LACERATION ON HIS RIGHT ARM CAUSED BY THE TRUCKSHIFT LEVER.
HE WAS HELPING ANOTHER EMPLOYEE HANG SOME CABLE WITH AN ARMADOR HIGH LIFT. HE WAS IN THE BASKET ABOUT 3 FT OFF OF THE GROUND.. THE MANLIFT STOPPED SUDDENLY AND HE HIT HIS RIGHT LEG ON MANLIFT BASKET AND BRUISED IT. THIS CAUSED A BLOOD CLOT IN THE MUSCLE.
EMPLOYEE WAS STANDING ON SILO SCALES WHEN WILEY SANDERS TRUCK BACKED ONTO SCALES. TRAILER HIT EMPLOYEE IN THE BACK KNOCKING HIM AWAY FROM TRAILER. AT THE TIME OF THE ACCIDENT THERE WAS NO INJU RY. ON JUNE 14, A993 EMPLOYEE'S DOCTOR TOOK HIM OFF WORK TO DO SURGERY ON HIS BACK. EMPLOYEE CLAIMS INJURY WAS CAUSED BY ACCIDENT ON 9-2-92.

EMPLOYEE WAS WORKING AT HALFWAY AS A BELTMAN. HE WAS STANDING AT THE CONTROL CABINET LOCATED ABOUT 25 FT FROM THE BELT HE WAS TIGHTING THE HYDRAULIC TAKEUP. SOMEWAY A ROCK WAS PINCHED OR THROW N FROM THE BELT AND HIT THE RIGHT SIDE OF HIS HAARD HAT AND BUSTED IT.
