FORKLIFT OPERATORS HARD HAT FELL OVER HIS EYES. WHEN ADJUSTING IT WHILE MOVING HE FOUND HIMSELF LEFT OF CENTER, HE TRIED TO AVOID CONCRETE FOOTER OF STEEL PILLAR. BY THE TIME HE APPLIED HIS BRAKE, HE WAS UP ON THE BASE AND SLOWLY ROLLED THE FORKLIFT OVER.

EE STOOD TOO CLOSE TO MOVING VEHICLE & HAD RIGHTFOOT BROKEN BY REAR WHEEL.
EE WAS SITTING IN THE INTERSECTION OF 45P & 44E ON THE GROUND FACINT SOUTH. EE WAS INSTALLING SURVEYING CENTER PINS. THE SCALER WAS TRAVELING SOUTH DOWN 45P WITH ALL EIGHT LIGHTS ON. SCALER OP ERATOR FAILED TO SEE SURVEYOR AND STRUCK HIM IN THE BACK, FOLDING HIM OVER AND UNDER THE SCALER. EE SUFFERED FRACTURE LEFT & RIGHT FEMUR, LEFT TIBIA & FIBULA, LEFT ANKLE.
EE WAS CLEANING TAIL PULLEY AREA OF ONE CONVEYORAND A CHUNK OF SALT FELL OFF OF ANOTHER CONVEYORBELT, STRIKING HER RIGHT SHOULDER.
21 MILL CONVEYOR STOPPED WHEN SPILLAGE WAS CARRIED ON THE RETURN SIDE OF THE BELT, TO THE TAIL PULLEY UNTIL THE PULLEY WAS CHOKED WITH SALT. THIS WOULD HAVE PUT TENSION ON THE TOP BELT. WITH P OWER LOCKED AND TAGGED OUT AND GUARDS REMOVED, EMPLOYEE WAS INVESTIGATING THE TAIL PULLEY WHEN THE TENSION RELEASED, AND RETURN SIDE OF BELT TRAVELED INTO PULLEY, PULLING LEFT ARM BETWEEN BEL
THE MEPLOYEE WAS SEATED IN THE PASSENGER SEAT OF A PICKUP TRUCK WHEN A SUPERVISOR BACKED INTO AN OUTRIGGER OF A PARKED ROOF BOLTER. THE EMPLOYEEWAS WEARING HIS HARD HAT, HE STRUCK HIS HEAD ON ROOF OF CAB. HE FELT THAT HE WAS NOT INJURED AT THE TIME, BUT AT THE END OF THE SHIFT HIS NECK AND BACK HAD STIFFENED UP.
EE RAISED COVER ON AUTOMATIC BAGGING LINE TO REMOVE JAMMED BAG. EE REACHED INTO THE BAG TAKE AWAY AREA AND CAUGHT LITTLE FINGER BETWEEN DRIVE BELT AND PULLY. EE DID NOT LOCK OUT EQUIPMENT.
EE WAS DRIVING SKID STEER LOADER OVER ROUGH TERRAIN. THIS CAUSED EE TO STRIKE ELBOW ON THE SIDE PANEL IN CAB. BECAME REPORTABLE ON 4/28/00.
