Showing all 38EE WAS OPERATING A SWAMP BUGGY AT THE #1 DRYING BED. THE EXACT CAUSE OF THE INCIDENT IS UNKNOWN AT THIS TIME. IT APPEARS THAT EE MAY HAVE OVERLOADED THE MACHINE, CAUSING IT TO OVERHEAT. THE RA DIATOR RUPTURED WHICH RESULTED IN EE BEING SPRAYED WITH HOT ANTIFREEZ. HE IS BEING TEATED FR BURNS TO HIS ARMS, LEGS, AND TORSO. NO EQUPMENT WAS FOUND TO BE DEFECTIVE OR MALFUNCTIONING.
EE WAS OPERATING #450 DEMPSTER DUMPSTER. WHEN HE RETURNED TO DISPATCH HIS CO-WORKER NOTICED HIS RIGHT EYE WAS RED. HE WENT TO EYEWASH STATION AND FLUSHED EYE. HE REPORTED THE INJURY ON 4-23-99 . HE WAS PRESCRIBED MEDICATION FOR HIS EYE. HE IS NOT LOSING ANY TIME AND IS NOT ON ANY RESTRICTIONS.
EMPLOYEE WAS WALKING IN THE MAIN AISLE OF THE MACHINE SHOP. AS HE APPROACHED THE METAL SHEAR, HE OBSERVED A SCOOTER MOIVNG TOT HE NORTH OF THE SHEA. THE PATH WAS RESTICTED BY A DELIVERY TRUC K. AS THE SCOOTER PULLED AWAY, A GUST OF WIND CAUSED DUST TO BLOW FROM THE BACK OF THE SCOOTER. HE TURNED HIS HEAD, BUT DUST ENTERED HIS RIGHT EYE. HE WAS WEARING SAFETY GLASSES WITH SIDE
EE REACHED UNDER CONVEYOR HEAD PULLEY GUARD TO DISCHARGE BOLT FROM HEAD PULLEY. EE'S SWEATSHIRTWAS PULLED INTO HEAD PULLEY AND HIS ARM CAUGHT ON THE END OF THE GUARD. THIS CAUSED A LARGE LACE RATION ON UPPER RIGHT ARM. EE SIGNALED CONTRACTOR WHO PULLED SAFETY LANYARD TO STOP BELT. EE IS NOT LOSING ANY TIME BUT IS ON WORK RESTRICTIONS DUE TO THIS INJURY
EE WAS OPERATING SMALL FRONT END LOADER UNDER #1TRAY THICKNER TANK. WHILE BACKING UP THE REAR TIRE RAN UP ON A CONCRETE CURB. THE CAB LIFTED AND STRUCK A DRAIN VALVE WITH HOT LIQUOR. THE LI QUOR SPRAYED ON THE LOADER AND THE OPERATOR. EE IS NOT LOSING ANY TIME AND IS NOT ON WORK RESTRICTINS.
EE WAS OPERATING AERIAL LIFT ON INCLINE AT 5 DOCK. UNEVEN SURFACE CAUSED AERIAL LIFT TO JERK EE WHILE IN BASKET. INCIDENT CAUSED EE TO STRAIN LOWER BACK MUSCLES. EE IS NOT LOSING ANY TIME AND IS NOT ON WORK RESTRICTIONS.
OPERATING CUSHMAN SCOOTER WHEN TIRE CONTACTED UNEVEN SURFACE. STEERING WHEEL SPUN AND STRUCK FINGER SPOKE. INITIAL X-RAY WAS NEGATIVE, FOLLOW UP X-RAY ON 3-2-98 REVEALED FRACTURE. EMPLOYEE IS NOT LOSING ANY TIME AND NOT ON WORK RESTRICTIONS.
HURT ELBOW WHEN TRACTOR HE WAS DRIVING HIT A MAN HOLE. EE WAS DIAGNOSED WITH TRAUMATIC BURSITIS OF THE LEFT ELBOW. EE IS NOT LOSING ANY TIME OR ON ANY RESTRICTIONS DUE TO THIS INCIDENT.
EE WAS WALKING NORTH IN NORTH B RECLAIM TUNNEL. HE HAD JUST STARTED THE CONVEYOR BELT WHEN HE TRIPPED ON A 2'X3' PILE OF BAUXITE. AS HE WAS FALLING, HE HIT A SCALING BAR THAT WAS LEANING AGAIN ST THE SCREW FEEDER. THE SCALING BAR STRUCK THE MOVING CONVEYOR AND LANDED BETWEEN THE BELTS ON THE RETURN SIDE. THE SCALING BAR SWUNG AROUND AND STRUCK EE ON THE R. KNEE. EE IS LOSING TIME D
EMPLOYEE WAS CHECKING PACKING ON 40-2 CAUSTIC CLEANING PUMP WITH HIS BACK TO THE ROAD. THE WATER TRUCK PASSED BEHIND HIM, WETTING HIM DOWN FROM THE KNEES DOWN. HE THOUGHT THE CAUSTIC CLEANING TANK HAD RUN OVER AND HE JUMPED AND FELT PAIN IN HIS MIDDLE BACK. EMPLOYEE IS NOT LOSING TIME DUE TO THIS INCIDENT, HOWEVER, HE IS ON WORK RESTRICTIONS.
FEE WAS OPERATING PAYLOADER AROUND #5 WASHER. HEFELT A JOLT ON LOWER BACK WHEN HE DROVE OVER STEEP RAMP & CAME DOWN. EE DID NOT LOSE TIME OR ON ANY WORK RESTRICTIONS DUE TO THIS INCIDENT. HOWE VER, ON 5/20/96, HE BEGAN RECEIVING PHYSICAL THERAPY AT THE PLANT MEDICAL DEPARTMENT.
EE STRAINED HIS BACK WHILE OPERATING A WALK BEHIND, BATTERY POWERED FORKLIFT. AS HE WAS BAKCING IT DOWN THE RAMP FROM THE SHOP, THE CHANGE IN GRADE CAUSED THE MACHINE TO LOSE ELECTRIC POWER AN D FREEWHEEL. HE THEN AGGREVATED HIS BACK WHEN CHANGING SHOES ON 10-27-95 AT 350 PM BEFORE GOING HOME. EE BEGAN LOSNG TIME DUE TO THIS INCIDENT ON 10-30-95.
EMPLOYEE HEARD AN UNUSUAL SOUND COMING FROM UNDER THE COWLING ON #175 TRACTOR. HE RAISED THE ENGINE COVER, STANDING ON THE LEFT SIDE OF THE TRACTOR AND LEANED OVER LISTENING TO THE MOTOR. THE BACK OF HIS LEFT HAND CAME IN CONTACT WITH THE FAN BLADES ON THE ALTERNATOR, LACERATING HIS KNUCKLES. EMPLOYEE IS NOT LOSING TIME DUE TO THIS INCIDENT, HOWEVER, SUTURES WERE REQUIRED
WHILE ATTEMPTING TO DROP OFF A SPIDER SCAFFOLD AT THE TOOLROOM, EMPLOYEE COULD NOT PULL FORWARD BECAUSE THE ISLE WAY WAS BLOCKED. HE BACKED OUT THE EAST DOOR, WENT ACROSS THE RAILROAD TRACKS A ND SIDE-SWIPED THE YELLOW BARRICADE PIPE. HIS LEFT FOOT WAS CAUGHT BETWEEN THE PIPE AND GO-DEVIL. EMPLOYEE BEGAN LOSING TIME 9-15-94.
EMPLOYEE WAS RUNNING A BOBCAT FRONT-END LOADER INSIDE #3 WASTER O'FLOW TANK. A LARGE MOUND OFMUD AND SCALE BROKE LOOSE FROM THE WALL OF HTE TANK AND PUSHED HIS LOADER ACROSS THE FLOOR OF THE TANK, PINNING IT AGAINST THE WALL. EMPLOYEE WAS COVERED WITH MUD AND SCALE.
EE WAS TRAVELING WEST LEAVING THE SAN PAT PLANT IN A CUSHMAN SCOOTER. HE FAILED TO YIELD TO ONCOMING TRAFFIC TRAVELING EAST TOWARDS SAN PAT WHEN HE TURNED TO THE SHERWIN PLANT ENTRANCE. EE SCO OTER AND AN ONCOMING CAR HIT AT THAT POINT.

AREA 24 DRIVING BOBCAT UP INCLINE, TIRE WENT FALT, STALLED OUT AND ROLLED BACKWARDS AND HIT SOUTH TRANSFER BELT. FELT PAIN IN NECK AND SHOULDER. ACCIDENT OCCURRED ON 1-15-91 REQURING NO MECI AL TREATMENT OR WORK RESTRICTION UNTIL DATE LISTED IN #6 ABOVE.

CONCRETE CREW WAS CHANGING THE BUCKET ON A 910 WALDON LOADER TO FORKS. DURING CHANGE, HIS LEFT INDEX FINGER WAS CAUGHT BETWEEN FORKS AFTER SLIGHT MOVEMENT OF EQUIPMENT CAUSING HIS INJURY. HE W AS TAKEN TO MEDICAL ARTS CLINIC FOR TREATMENT. DIAGNOSIS: COMMINUTED CLOSED FRACTURE OF DISTAL PHALANX (LEFT INDEX FINGER), POSSIBLE JOINT INVOLVEMENT.
