Teammate initially reported being sick, but later disclosed an injury. They stated a foot was hanging off the forklift, and while traveling down a bay, leg was caught between the forklift and a pallet, injuring their ankle. They received medical attention and was diagnosed with a strained left foot and a pulled muscle.
While transporting a line with a forklift, an EE got debris in EE's eye, possibly from rubbing it. EE used the on-site eyewash station and reported it to EE's supervisor, who escalated the issue. After initial first aid, the EE was taken to a fire station, then to Archbold Corp. Care for further treatment and received prescription medication.
EE was complaining about another teammate not loading EE easy. EE never indicated EE was injured during the process but came back later in the week and wanted medical.
EE's (L)forearm injury occurred when bales got stuck on the conveyor. EE came to clear the jam. EE picked up the bale and laid left hand down on the exit conveyor. The exit conveyor then carried EE's hand the head pulley where EE's hand became trapped. Given 1st aid for burning, medical attention, meds and released back to work with light duty, (L) arm until further notice.
Employee was backing out of the warehouse with a bulkbag. They turned the lift toward the north. Another employee was standing beside another lift with their back to first employee. The forklift employee was driving came into contact with the stationary forklift and then glanced off onto Injured employee's right leg causing a contusion.
Repositioning a bag on conveyor when conveyor belt caught glove & pulled right hand down into gap between head pulley & idler roller. Diagnosis - crush, deep abrasion/burn. Restrictions: must keep dressing clean & dry, no lifting, no use of right hand, ice hand & no working near hazardous or moving machinery, of driving & climbing due to medication.
Operator backed against stockpile and material was unable to dump properly. Assumption is material may have shifted bending the hydraulic cylinder and haul truck then slowly laid on its side. Diagnosis is transverse process fractures.
Portion of wooden pallet struck Forklift Operator in mouth. Caused laceration to left upper lip of mouth. Laceration required sutures. No restrictions.
EE bending down picking up cardboard and forklift carrying pallet struck her right hip & elbow causing same level fall. EE didn't want to see doctor. After 2 days had soreness & requested to see doctor. Diagnosis: body trauma, no fractures or broken bones. This is lost time due to medication prescribed and not the actual injury.
Employee was moving forks together on a forklift. Hand slipped while moving fork over, fork came down and pinched employees right thumb causing a laceration.
EE attempting to sling motor onto loadall forks. Sling slid along forks pinching left ring finger between motor & handrail. Laceration required sutures. Restrictions, limited use of left hand, keep dressing clean & dry. Recheck in 1 week.
EE was loading a bulk truck; when for reasons unknown, the driver removed wheel chucks & drove away from loading point without being instructed to do so by EE. The retractable lanyard swung EE off truck. EE's main complaint was pain in areas consistant with full body harness.
No direct injury. Her back was hurting and she thought it was from the seat on the loader. No direct cause has been determined. Diagnosis was lower back spasm. Restricted no lifting over 15 lbs, no driving loader, no prolonged standing, bending or stooping.
Forklift driver pushed conveyor into EE's right leg was caught in between the seat he was sitting at and the conveyor.
Backing skid steer and hit a front end loader. Diagnosis mild cervical and lumbar strain with no restrictions. Sent back to doctor with mild pain on 4-22-10 and was referred for physical therapy. Became reportable on 4-22-10.
Roderick was driving forklift moving a pallet when dust entered left eye. He was wearing safety glasses. Diagnosis: scleral abrasion and medication was prescribed. He had restrictions, but is back to normal duties.
Injured employee was helping install a new roll of plastic wrap on a wrapping machine. Employee failed to comply with lockout/tagout policy and procedures. He stepped up onto a conveyor and tripped a photo eye which started the conveyor. Employee's foot was caught between the conveyor and the wrapping machine feed conveyor. He received minor scrape and bruise to right ankle.
Cleaning railroad tracks with a Bobcat and hit tracks with bucket on Bobcat. The impact is what hurt his left shoulder. Did not loose any time from work and no restricted duty. Shoulder still hurting, so doctor sent him for physical therapy starting 8-16-06.
CLEANING UP WITH BOBCAT, PICKED UP HEAVY DISK, TRIED TO ARRANGE DISK ON BOBCAT AND INJURED LOWER BACK. (HE IS STILL WORKING BASED ON HIS WORK RESTRICTIONS.)
ABRASION TO ARM REACHING FOR BAG, WHEN IN CONTACT WITH A PACKAGING CONVEYOR BELT, HE IS WORKING RESTRICTED DUTY.
Employee received minor back strain while operating wheeled front-end loader. Employee stated that seat shifted when entering the stockpile.
Forklift had plastic caught on forklift, forkilft traveling past ee and it caught ee's leg causing him to fall down.
Employee received bruise and slight abrasion to left shoulder as a result of a forklift accident inside a 18-wheel trailer.
EE WAS BACKING LOADER OUT OF DESINTIGRATOR FEED HOPPER AND HIT A CONCRETE BEAM.
EE WAS CLEANING FROM UNDER THE CONVEYOR, PRODUCTION LINE. EE PLACED HIS LEFT HAND ON CONVEYOR RAILING, THERE WAS A PALLET TRAVELING DOWN THE CONVEYOR. THE PALLET HIT EE'S LEFT HAND, INJURING H IS LEFT HAND.
CONVEYOR BELT WAS SLIPPING OFF ROLLER AND TRIED TO PUSH BELT BACK ON ROLLERS WITHOUT TURNING POWER OFF MACHINERY.
EMPLOYEE WAS STANDING ON A CONVEYER RESTACKING A PALLET WHEN HE STARTED UP CAUSING HIS FOOT TO GET CAUGHT BETWEEN TWO CONVEYERS. THE EMPLOYEE LEFT ANKLE WAS INJURED AND HE WAS IMMEDIATELY TAKEN FOR MEDICAL TREATMENT.
EMPLOYEE WAS PLACING POLYCAPS COVERS ON PALLETS THAT WERE TO BE CARRIED BY TROLLEY CAR TO BE STRETCH WRAPPED. SHE WAS AWARE OF THE TROLLEY CAR COMING DOWN THE TRACK AND ATTEMPTED TO PLACE A P OLYCAP ON BEFORE THE TROLLEY CAR RETURNED. SHE MISTIMED THE TROLLEY AND GOT HER RIGHT LEG CAUGHT BETWEEN THE TROLLEY CAR AND CONVEYER. SHE IS A TEMP EMPLOYEE WITH 2 WEEKS EXPERIENCE IN JOB.
