EMP USED FOOT B9AKE AND RETARDER BRAKE TO STOP VEHICLE DUE TO HIS BELIEVING HE WAS GOING OVER THE JAW CRUSHER DRAMP BERM. THE SUDDEN STOPPING OF VEHICLE DUE TO HIS USING BOTH BRAKES AT SLOW SP SPEED AND HIS A5READY BEING LEANED FORWARD WHILE DOING SO CAUSED HIM TO CONTINUE HIS FORWARD MOTION RESULTING IN MINOR NOSE BLED FROM CONTACT WITH STEERING WHEEL RIM AND MUSCULAR TRAUMA FROM
EMPLOYEE HAD MU8CLES IN THE LOWER BACK PULLED WHILE OPERATING THE KOMATSU LOADER IN THE BLAST PILE.

EMP WAS OPERATI3G A LOADER. A HYDRAULIC HOSE BROKE AND THE WINDSHIELD WAS BROKEN OUT LETTING HYDRAULIC FLUID INTO THE OPERATORS CAB HE PANICKED AND JUMPED FROM THE LOADER FALLING 12' TO THE GR OUND. STAYING I3 THE CAB WOULD HAVE RESULTED IN VERY MINOR INJURIES
EE WAS ATTEMPTING TO EMPTY R/R CARS. HE ATTACHED A CHAIN TO THE BOTTOM DOOR, RAN THE CHAIN THRU A LOOP ON THE CAR BODY THEN WRAPPED THE CHAIN AROUND HIS LEFT HAND. AS THE TRAIN MOVED FORWA RD THE DOOR SWUNG OPEN PULLING HIS HAND THRU THE LOOP.
WETTING DOWN DUMPING AREA.TRIED TO BACK TRUCK AND WENT OFF ROAD BANK-WATER SHIFTED AND WEIGHT CAUSED TRUCK TO SLID OVER ON ITS SIDE.
