Showing all 5The mechanic was attempting to reinstall a lift cylinder on an articulating dump truck in the parking area at Holliday Rock Jobsite. The strap that was attached to the crane and the cylinder slipped causing the cylinder to fall and strike the mechanic on the right-side causing accident to ribs and right arm.
Wet roads, and employee lost control of haul truck and ran into side of embankment.
While monitoring the pit line, employee stood up and turned swinging hand into roller, employee's hand made contact with troughing roller. Roller pulled arm in up to elbow. Employee pulled arm out on own.
While working on loader, Injury was operating the truck mounted crane, when it moved unexpectedly, pushing a metal plate that he was holding and catching his left ring finger between the metal plate and bolt, pinching his finger.
