WHILE OPERATING A WATER TRUCK UP A RAMP IN THE QUARRY, THE EMPLOYEE "BLACKED-OUT", CAUSING THE TRUCK TO CLIMB OVER THE BERM, DESCEND A 25-FOOT HIGH EMBANKMENT AND OVERTURN ONTO ITS SIDE ON A H AUL ROAD BELOW.
THE EMPLOYEE POSITIONED HIS DUMP TRUCK UNDERNEATH A BIN AND APPLIED THE TROLLEY BROKE BEFORE OPENING THE TRUCK DOOR TO OPEN THE BIN. WHILE LEANING OUT OF THE CAB, HIS KNEE BUMPED THE TROLLEY B RAKE HANDLE CAUSING THE TRUCK TO DRIFT BACKWARDS. HIS SHOULDER BECAME PINCHED BETWEEN A STEEL BEAM AND THE TRUCK DOOR.
AS A LOADER WAS DESCENDING THE WASHED SAND STOCKPILE, THE BUCKET CAUGHT THE GROUND AND THREW THE OPERATOR FORWARD INTO THE WINDSHIELD.
