When dumping the load, the driver felt a twinge in lower back when the driver twisted. Doctor stated strain, and advised one week of no truck driving.

EE WAS OPERATING A SMALL BOBCAT FRONT END LOADER. WHILE DOING SO THE EE WAS HAVING TO FREQUENTLY TWIST HIS TORSO TO LOOK OVER HIS SHOULDER TO SEE WHERE HE WAS GOING AS HE BACKED THE FRONT END LOADER. THIS REPEATED TWISTING AGGREVATED A PREVIOUS SHOULDER INJURY. THE EE REPORTED THE INJURY 12/08/03. HE WAS PLACED ON MODIFIED DUTY ON 12/15/03.
THE EE WAS DRIVING A SMALL FRONT END LOADER SCRAPING MATERIAL OFF OF THE FLOOR. THE BUCKET OF THE LOADER CAUGHT ON A PIECE OF RE-BAR ROD STICKING UP FROM THE GROUND WHICH CAUSED THE LOADER TO STOP ABRUPTLY. THE EE WAS JARRED BY THE SUDDEN STOP.
EMPLOYEE WAS WALKING BESIDE FINISH MILL BUILDING WHICH IMPAIRED HIS ABILITY TO HEAR APPROACHING TRAFFIC. EMPLOYEE TRAVELING APPROX 5 MPH IN A PICKUP TRUCK STRUCK THE EMPLOYEE WITH THE TRUCK FR OM BEHIND. INJURIES WERE MINOR AND INCLUDED A BRUISED ELBOW, SORE BACK, AND TWISTED KNEE.
EMPLOYEE WAS TRYING TO REMOVE A REMOVABLE HANDRAIL WITH ANOTHER EE USING A FORKLIFT. THE FORKLIFT WAS TOO SHORT SO THE EE PUT A PIECE OF ANGLE IRON BETWEEN THE HANDRAIL AND THE FORKS OF THE FO RKLIFT. THE ANGLR IRON WAS NOT SECURED AND THE PRESSURE BETWEEN THE FORKLIFT AND HANDRAIL CAUSEDTHE ANGLE IRON TO SHOOT OUT TOWARDS THE EE. THE ANGLE IRON MADE A DEEP CUT ON RIGHT MIDDLE FING
EE WAS INSTALLING A FAN PEDESTAL BLOCK WHILE LIFTING PEDESTAL BLOCK WITH FORKLIFT AND CHOKER THE CHOKER SLIPPED OFF FORKS CAUSING WEIGHT SHIFT, AND KNOCKING EE DOWN.
