THE EE WAS RIDING AS A PASSENGER ON A LOCI LOOKING FOR A WATER VALVE. HE WAS LOOKING IN THE OPPOSITE DIRECTION OF TRAVEL. AS THEY WERE TRAVELING DOWN THE DRIFT, THE EE STRUCK HIS HEAD ON A PIE CE OF VENTILING THAT WAS HANGING LOW.
EMPLOYEE WAS DRIVING A BUGGY DOWN THE 46E RAMP WHEN THE TRANSMISSION BROKE. THE EMPLOYEE DROVE THE BUGGY INTO THE RIB. HE STRUCK HIS WRIST (RIGHT) ON THE STEERING WHEEL.
EMPLOYEE REPORTED THAT HIS BACK WAS SORE. HE HAD BEEN DRIVING A HAUL TRUCK AS HE NORMALLY DOES.
EMPLOYEE WAS RIDING IN A BUGGY DOWN TO HIS WORK AREA. THE BUGGY BOUNCED WHEN IT RAN OVER A ROCK AND TEH EMPLOYEE STRUCK HIS BACK ON THE SIDE RAILINGS.
EMP WAS MUCKING A SLAB ROUND WITHA RAIL MUCKER. AS HE BACKED UP TO LOAD THE ORE CAR, HE RAN INTOA MAT THAT WAS HANGING OFF THE BACK AND CUT HIS RIGHT EAR. HIS REFLEXES CAUSED HIM TO LUNGE FORW ARD AND HE HIT FOREHEAD ON BUCKET OF MUCKER.
EMP WAS RIDING INTO THE 500W PORTAL IN THE LEAD CAR WHEN IT DERAILED. THE SAFETY CHAIN WAS NOT LATCHED, AND WHEN THE CAR LEFT THE TRACKS IT OPENED FAR ENOUGH THAT THE EMP FELL THROUGH. IT CLOS ED ON BOTH HIS LEGS BELOW THE KNEES. EMP FAILED TO FOLLOW ESTABLISHED SAFETY PROCEDURES BY RIDING IN THE LEAD CAR AND NOT FASTENING CHAIN.
EMP REPORTED THAT HER BACK AND SHOULDERS WERE SORE AND ACHING AFTER DRIVING TRUCK UNDERGROUND DURING HER SHIFT ON 9-20-95. NO SPECIFIC INCIDENT WAS MENTIONED. EMP REPORTED THIS ON 9-23-95. SHE WENT TO THE DR ON 9-25 & WAS PLACED ON RESTR.

THE EMPLOYEE FAILED TO RETRACT THE CAMEL DUMP AFTER DUMPING A LOAD OF WASTE.HE HAD CLIMBED INTO THE SECOND CAR AND SIGNALLED THE OPERATOR TO COME BACK.AS THE CAR WENT OVER THE DUMP IT OPENED T HE CAR AND THE EMPLOYEE FELL THROUGH.THE CAR CLOSED PINNING HIS LEGS JUST BELOW HIS KNEES.HE SUSTAINED CUTS AND BRUISES AND WAS RELEASED FOR LIGHT DUTY WORK FOR 48 HOURS BUT DID NOT REUTRN UN
THE EMPLOYEE WAS SITTING IN THE BACK OF A BUGGY. AS THE BUGGY BEGAN TO MOVE THE CABLE FROM A COMNE-ALONG THAT WAS IN THE BACK OF THE BUGGY WENT THROUGH A HOLE IN THE FLOOR, WRAPPED AROUND THE DRIVE LINE U-JOINT & PULLED THE CABLE TIGHT AROUND THE EMPLOYEE'S RIGHT FOOT CAUSING A LACERATION IN THE FOOT.
EE WAS HAULING 22X10 FIBERGLASS VENTLINE USING A WAGNER 2 YEAR MUCKER. THE VENTLINE WAS TIED OVER THE WHEELS WELLS WITH THE OTHER END ON THE MUCKER BUCKET. THE EE WAS LEANING OUT OF HIS SEAT T O GAIN VISIBILITY AROUND THE VENTILINE ONE OF THE SECTION OF VENTLINE CAUGHT ON EXISTING VENTLINE AND PUSHED INTO HIS LEFT RIB.
INJURED AND EMPLOYEE WERE GOING TO THE WEST MUCKBAY TO FUEL A 3.5 4D MUCKER. WHILE WAITING FOR THE MUCKER AT THE MUCK BAY, EMPLOYEE BACKED UP THE PICKUP SO THE MUCKER OPERATOR COULD PUL INTO THE MUCK BAY. WHILE PICK UP WAS BACKING UP, INJURED OPENED THE DOOR CATCHING HIS HAND BETWEEN THE DOOR AND RIB.
EMPLOYEE WAS CLEANING UP UNDER A CHUTE WITH A 1 YD MUCKER. THE CHTUE GATE HAD A ROCK STUCK IN IT WHICH ALLOWED ANOTHER ROCK TO FALL OUT OF THE CHUTE, STRIKING HIM ON THE LEFT FOREARM.
EMPLOYEE WAS DRIVING A BUGGY, HIT A HOLE IN THE ROAD AND STRAINED HIS BACK.
EMPLOYEE HAD A CHAIN HOOKED TO THE DIPPER MOTOR SPROCKET THEN TO A OFF CENTER EYE PIN IN THE BACK TO LIFT THE COPCO WHICH WAS STUCK WHEN HE LIFTED THE COPCO IT SWUNG OVER PINNING HIM BETWEEN T HE COPCO & HANGING WALL
WHILE MUCKING WITH A 5 YD. MUCKER, EE HIT A BUMP IN THE ROADWAY. HE CAME DOWN ON THE SEAT HARD AND FELT A SEVERE PAIN BETWEEN HIS SHOULDER BLADES.
EE GOT DUST IN HIS EYE. WHILE OPERATING A LOCOMOTIVE THAT WAS BEING UTLIZED TO PULL ANOTHER LOCOMOTIVE OUT FROM UNDERGROUND.
DELIVERING SUPPLIES ON A BUGGY, & JAMMED HIS BACK ON ROUGH ROAD.
AS EMPLOYEE WAS TRAMMING MUCK FROM A MUCK BAY WITH A LHD 2 YRD MUCKER HE HIT A ROCK CAUSING THE MUCKER TO SWERVE INTO THE RIB STRIKING HIS LEFT ARM ON A GROUND SUPPORT MAT.
EMPLOYEE WAS ATTEMPTING TO DROP HITCH PININTO A PIECE OF RAIL EQUIPMENT. WHEN THE LOC. MOVED HIS HAND WAS PINCHED BETWEEN THE PIN AND LOCI.
OPERATING AN LHD COMPLAINED OF SORE BACK HE CONTINUED TO WORK UNTIL 01-13-93 & CONSULTED A PHYSICIAN WHO PULLED HIM OFF WORK AT THE TIME THE STATUS & CLASSIFICATION OF THE INJURY WAS KNOWN
38503HUTE IN HIS STOPE HAD BEEN PULLED EMPTY. HE WENT DOWN THE MANWAY TO CLEAN UP AT THE BOTTOM. HIS PARTNER DUMPED A LOAD OF MUCK DOWN THE CHUTE. THE ROCK BOUNCED OVER THE GATE AND STRUCK 54736YEE ON THE FOOT.
EMPLOYEE WAS MUCKING STOPE AND HAULING MUCK TO MUCK PASS, HE HAD HAULED APPROX 17 LOADS TO THE MUCK PASS ABOUT 640 AWAY WHEN HE COMPLAINED OF PAIN AND NUMBNESS IN HIS LEFT ARM.
THE EE WAS STRUCK BY 3 INCH SANDLINE THAT CROSSED THE DRIFT HE HAD INSTALLED THE LINE 2 HOURS BEFORE THE ACCIDENT HE WAS RIDING STANDING IN THE LOEAD CAR OF A MACK TRAINING BEING PUSHED INTO T HE MINE. THE PIPE STRUCK HIS HAND HAT WHICH PUSHED HIS SAFETY GLASSES INTO HIS NOSE AND FOREHEAD.
EMPLOYEE WAS SETTING TIES BENETH TRACK IN PREPARATION OF A RAILROAD SWITCH. A TRAIN OPERATOR WHO WAS HAULING MUCK IN THE AREA WENT TO MOVE THE TRAIN TO ALLOW A BUGGY TO PASS AND STRUCK THE EMP LOYEE WITH THE LEAD CAR OF THE TRAIN. THE VICTIMWAS PINNED BY THE CAR.
DRIVING LHD DOWN HAULAGE TO THE FACE WHEN HE HIT A POT HOLE WHICH CAUSED THE LHD TO VEER INTO THE DRAIN DITCH. HE HAD HIS LEFT HAND ON THE SEAT FRAME AND IT WAS PINCHED BETWEEN THE FRAME AND T HE RIB.
PINNING RAIL UNDER TUNNEL BORING MACHINE AND ROCK ROLLED OFF THE CONVEYOR BELT AND STRUCK HIM ON THE RIGHT HAND.
