EMPLOYEE WAS CHECKING TAILING AREA DRIVING JEEP 4X4 #57165. HE WAS DRIVING AROUND #3 EVAPORATION POND HE VEERED OVER ONTO THE EAST SHOULDER OF ROAD AND ANGL ED DOWN SIDE OF BANK ABOVE SEEPAGE DITCH. THE JEEP ROLLED OVER ONE COMPLETE TURN, LANDING ON ITS WHEELS IN BOTTOM OF SEEPAGE DITCH CAUSING INJURY TO EMPLOYEES BACK AND CUT ON FOREHEAD
EMP WAS HELPING LOAD A BUNDLE OF TIMBER IN THE CAGE. THE BUNDLE WAS SUSPENDED IN A HORIZONTAL POSITION FROM THE BACK OF THE WINCH TRUCK. WHILE TRYING TO GUIDE THE TIMBER INTO THE CAGE THE BUND LE SWUNG HORIZONTALY AND PINCHED THE EMPLOYEES HAND AGAINST ONE OF THE WINCH TRUCK BOOMS
THE EMPLOYEE WAS DRIVING A MOTOR PUSHING A SUPPLY CAR BACK INTO THE MINE, THE SUPPLY CAR WAS LOADED WITH WIRE MESH AND TIMBER. HE WAS PASSING THROUGH A NARROW AREA. A PIECE OF WIRE MESH WAS PR OTRUDING FROM THE RIB. IT HOOKED ONE OF THE ROLLS OF WIRE ON THE SUPPLY CAR AND THE ROLL OF WIRE WAS FLIPPING ON THE CAR AND STRUCK THE MOTOR OPERATOR.

THE DRIFT MINER AND HELPER WERE GOING INTO THE FACE OF HAULAGE DRIFT ON MOTOR HELPER WAS RIDING IN BACK OF MOTOR GOING BY 5502 SW COMPARTMENT THE TUGGER CABLE WAS TOO FAR DOWN AND LAYING ON TR ACK THE MOTOR CAUGHT CABLE PULLED IT INTO THE TIMBER SET PULLING IT DOWN CAUSING THE LAGGING FROM THE TIMBER SET TO STRIKE THE HELPER IN THE HEAD AND SHOULDER HELPER RECEIVED PAIN IN LOWER BA
THE EMPLOYEE WAS ON TOP OF THE #320 MOTOR ADDING HYDRAULIC FLUID. THE MOTOR WAS PARKED AT THE ENTRANCE TO THE SHOP AND THERE WERE TWO MOTORS PARKED BEHIND IT. ANOTHER EMPLOYEE STARTED A MOTOR BEHIND HISAND IT ROLLED INTO THE 320 MOTOR CAUSING PADILLA TO LOSE HIS BALANCE AND FALL TO THE GROUND, BRUISING HIS LEFT KNEE.

TOBY PADILLA, HAULAGE DRIFT HELPER WAS GOING TO 8100 DRIFT FROM THE STATION WITH 5 EMPTY ORE CARS & #319 DIESEL MOTOR. THE COLLISION BETWEEN THE ORE CARS & #320 MOTOR HAPPENED ON THE CURVE NEAR THE MECHANIC SHOP, ABOVE 200\ FROM THE 0200 TRAFFIC SIGNAL KNOCKER. BOTH MEN CLA
