EMPLOYEE WAS OPERATING LUBE TRUCK GOING TO THE TAILINGS DAM TO START EQUIPMENT FOR THE DAY SHIFT PRODUCTION PEOPLE.AFTER STARTING DOWN THE ROAD TO THE DAM THE EMPLOYEE FELT THE BRAKES WERE NOT HOLDING WELL ENOUGH SO HE DOWN SHIFTED BUT MISSED THE NEXT LOWER GEAR. CONSEQUENTLY THE VEHICLEGAINED SPEED AND WAS NOT ABLE TO BE STOPPED BEFORE GOING OVER THE BERM.

INJ IS MINE SUP9RINTENDANT AND WAS ESCORTING A SNOW BLOWER WHILE REMOVING SNOW FROM THE MAIN ROAD. INJ WAS COMPLETELY STOPPED WHEN HE WAS HIT FROM BEHIND BY ANOTHER VEHICLE. THE IMPACT CAUSED A MINOR WHIP LA8H WHICH THE INJURED WENT TO A CHIROPRACTOR FOR ADJUSTMENT. THIS WHIPLASH WAS A NAN AGGRAVATION OF A PREEXISTING CONDITION WHICH HAD BEEN TREATED ON A CONTINUING BASIS
