WITH THE VACUUM TRUCK RUNNING, SUCTION ON EE WAS ATTEMPTING TO ADD A LENGTH OF HOSE TO THE ONE ON THE TRUCK. AN OLD PIECE OF DUCT TAPE WAS HANGING DOWN AND WOULD INTERFERE WITH THE CONNECTION SO HE REACHED UP TO REMOVE IT AND HIS LEFT ARM AND SHOULDER WERE SUCKINTO THE HOSE AND THE HOSE SEALED ITSELF AGAINST HIS BODY AROUND THE SHOULER. SUCTION CONTINUED FOR APPROX ONE MIN.

EMPLOYEE WAS KNEELING OVER THE NORTH SIDE OF LEAD TRACK AT FROG, ARC WELDING THE FROG. THE AIR BLED OFF OF RAIL CAR BRAKES AND CARS ROLLED APPROX. 164 FT TO WHERE EMPLOYEE WAS WORKING THE AXL E SHOUING ON THE NORTH SIDE OF THE AXLE CLOSET TO HIM PUSHED HIM TO THE NORTH WEST AWAY FROM THE RAIL HIS RIGHT HAND WAS CAUGHT RAIL CARS WHICH WERE LOADED.
WHILE CLEANING THE WAREHOUSE, HE WAS HIT FROM BEHIND BY A FORKLIFT.
EMPLOYEE WAS TA5ING THE COVER OFF OF B-2-1C1 HYDRAULIC COUPLING BELOW THE OVERHEAD CRANE.ANOTHER EMPLOYEE,OPERATING THE CRANE,LOWERING A SWITCHBOX HOOKED ONTO A CABLE,RESTED THE CABLE ON THE H ANDRAIL OF B2-171 AND LET OUT THE SLACK,WHEN THE CABLE WAS RELEASED,IT SLIPPED AND SWUNG AROUND TO THE WEST SIDE WHERE I NAME WAS WORKING AND HIT HIM ON THE LEFT SIDE OF HIS HARD HAT.
WHILE DUMPING O3L DURMS FROM A PALLET ON A FORKLIFT HE HAD HIS R HAND ON THE BRACE IN FRONT OF FORKLIFT. ANOTHER EMPLOYEE WAS TRYING TO TILT THE FORKLIFT FORK DOWN BUT ACCIDENTLY MOVED THE S IDE SLIDE LEVER3AND HIS FINGERS GOT CAUGHT BETWEEN FORKS AND FORK BRACE
EMPL LIFTED BAR9EL OF MILL BALLS UNDER PEPPER TREE AND BRANCH GOT HUNG UP ON FORKLIFT EXTENSION,STARTED TO LOWER FORKS TO BARREL.TREE BRANCH BROKE AND IT MADE FORKS DROP AT SAME TIME EMPL WAS PUTTING BARREL3GRABBER ON BARREL,CAUGHT HAND BAETWEEN FORKS BARREL GRABBERS.
AS THE EE WAS 4ANEUVERING THE LAB SAMPLER TRUCK IN A FORWARD DIRECTION TO T HE KILN FEED SAMPLE POINT HE THOUGHT HE HEARD SOMEONE CALLING HIM. AS HE TURNED HIS HEAD TO LOOK OVER HIS LEFT SHO ULDER THE TRUCK3STRUCK A VERTICLE SUPPORT COLUMN FOR THE AIR SLIDE CATWALK BETWWEN THE T2-4 BLDG & THE KILN FEED SILOS. THIS CAUSED THE EES HEAD TO STRIKE & BREAK THE WINDSHIELD HIGH ON THE
EMPL DRIVING QU5RRY TRUCK UP INCLINING ROAD,DRIVE SHAFT BROKE.ALSO AN AIR VAVLE BROKE.IT CAUSED FOOT BRAKE AND RETARDER BRAKE TO FAIL.TRUCK ROLLED BACK WITH DRIVER LOSING CONTROL OF STEERING A ND WENT OVER 492 EMBANKMENT.
WHILE DRIVING I3TO PLANT PARKING LOT, EMPLOYEE RAN INTO THE 454 FRONTEND LOADER, STRIKING LOADER S FRONT WHEEL. EMPLOYEE STRUCK HIS LEFT KNEE AGAINST CB RADIO, AND HIS HEAD HIT THE FRONT WINDS IELD. EMPLOYEE 9EPORTEDLY WAS DRIVING TO WORK.
