Employee was leaving National Quarry driving to Crown Quarry. He was crossing IN 37 South, the truck side post obstructed his view and he pulled out into the path of another vehicle, striking him in the passenger side.

EE WAS HAULING A LOAD, FAILED TO SEE A LARGE MOUND IN THE AREA HE WAS WORKING. WHEN THE LOAD STRUCK THE MOUND, IT CAUSED THE LOADER TO JAR, THROWING HIM FORWARD AND CAUSING HIS LOWER BACK TO H URT.
