USED A SKID STEER LOADER TO LOWER LARGE HYDRAULIC JACKS TO THE GROUND. REPAIRMAN SIGNALLED OPERATOR TO TAKE UP SLACK IN CABLE. OPERATOR DID NOT STOP SOON ENOUGH AND JACK CABLE WAS PULLED INTO THE SHEAVE. HIS FINGER WAS PINCHED BETWEEN SHEAVE AND JACK.

EMPLOYEE WAS OPERATING SMALL SKID STEER LOADER. DISCHARGED A BUCKET FULL OF LIME AND SOME DUST GOT INTO RIGHT EYE. WAS NOT WEARING HIS SAFETY GLASSES.
