Showing all 6Employee was checking on a squeaky roller while roller was in operation. Left hand got caught in roller and tore the skin on the left wrist in two places.
Drove over a couple of ruts in the yard causing the loader to bounce fairly hard. Ruts have since been filled in.
Alleged back strain from loader seat, loader hit a bump.
Employee was operating a CAT 980 C loader. As the machine dug into the bank of material, the seat failed (later found to be safety wired) and the employee fell backwards.
