EMPLOYEE COMPLAINED OF A SORE NECK AT THE START OF SHIFT ON 11/14/90. THE ONLY TIME SHE COULD RECALL BEING JARRED AROUND WAS ON 11/14/90 WHEN THE BUCKET OF THE LOADER HIT HER TRUCK WHILE LOADE R ON THE EXACT TIME OF THE DAY BUT THOUGHT IT WAS EARLY IN THE SHIFT.
EMPLOYEE WAS OPERATING FRONT END LOADER TRUCKS FROM 09-16-90 ROUGH RIDE OF LOADER APPEARS TO HAVE AGGRAVATED AN EXISTING BACK CONDITION EMPLOYEE DID NOT REPORT PAIN UNTIL 7 DAYS AFTER FIRST PA IN APPEARED EMPLYEE HAD BEEN OFF FOR 2 DAYS PRIOR TO REPORTING THE INJURY & CALLED IN PRIOR TO START OF SHIFT THIS MAY NOT BE A JOB RELATED INJURY INJURY IS BEING REPORTED DUE TO THERAPY TREA
EMPLOYEE WAS DRIVING HAUL TRUCK INTO PIT WHEN HE LOST DYNAMIC RETARDATION. EMPLOYEE STOPPED TRUCK BY DRIVING INTO A MUCK PILE AT THE BOTTOM OF PIT. EMPLOYEE CONTINUED WORKING TILL AST TWO DAYS OF SCHEDULED SHIFT (7-10-90) AT WHICH TIME HE WENT TO A DR. HE RETURNED TO NEXT SCHEDULED WORK SHIFT (7-14-90). DR. DIAGNOSIS WAS A STRAIN TO THE CERVICAL-THORACIA AREA.
ACCIDNET HAPPENED ON THE 5540 LEVEL IN THE SOUTH PIT. 207-1 LOADER WAS LOADING WASTE MATERIAL INTO 71-7 HAUL TRUCK AND DRIVER WAS JARRED WHEN THE FIRST LOAD OF MATERIAL WAS LOADED INTO THE TRU CK.
#73-7 TRUCK WAS LOADED ON 5540 BENCH 2 BUCKETS OF LOOSE MAT'L WERE LOADED, THEN A LARGE BOULDER WAS LOADED ON REAR AND PUSHED OVER REAR AXLES. TRUCK PROCEEDED UP 11-15% RAMP. FRONT END CAME OF F GROUND DRIVER EASED OFF THROTTLE. TRUCK CAME BACK DOWN DRIVER HIT THROTTLE AGAIN THROWING TRUCK TO POINT WHERE IT WAS RESTING ON REAR OF BED AND BUTT OF TRUCK (VERTICAL TO GROUND) DRIVER RE
EMPLOYEE ALEDGES HE HIT HIS BACK ON FIRE SUPRESSANT CONTROL BOX WHILE BEIGN TRAINED ON LOADER. SPRAINED IN THE MUSCLES IN HIS LOWER BACK.
