Truck driver neede 1000 # sand. He was instructed to exit scales and wait for loader to bring the sand. EE drove to the sand load out area instead. He then went to the sand rile, did a U-turn and pulled behind the loader as it was backing up. The loader hit the right side and damage was done to the truck, loader not damaged. No mining imparment as result of accident.

MINER WAS CLEARING HAUL ROAD WITH A 1996 CAT LOADER. THE BUCKET DUG INTO ROAD CAUSING THE LOADER TO SUDDENLY STOP, THROWING THE MINER INTOTHE WINDSHIELD. HE WAS NOT WEARING THE SEAT BELT . HE HAD BEEN INSTRUCTED AND TRAINED TO WEAR THESEAT BELT AND OPERATE LOADER SAFELY.
