EE was using a Bobcat walk behind loader for clean up when his left hand was pinched between the machine and the cement wall. He was evaluated by a Physician after the event and diagnosed with a fracture to his left middle finger. He took himself off for his next scheduled shift due to the need for pain medication. He has returned to work.

He drove into the pile & when lifting the bucket, the back of the machine lifted up and pinched the left hand. He was evaluated by a Physician; diagnosed with a laceration and fracture to left pinky finger. He was given restrictions & returned to work.
A production truck driver reported of being jostled while his truck was being loaded, causing back pain. He was evaluated by a physician and given Rx narcotics & muscle relaxant. He was released with restrictions, but unable to report for his 4/2 shift due taking the medications. He returned light duty for his 4/3 shift, was scheduled off 4/4-4/7 & released full duty as of 4/8.
Driver was dumping a load of Coal and while the box was raising it became unstable due to frozen coal stuck in the trailer and an uneven surface which caused the truck and trailer to tip over.
The employee was operating a fork lift when he hit a launder and the forklift stopped suddenly. As a result, the employee hit his mouth on the steering wheel and sustained a laceration to his lower gum. He was evaluated by a physician on 12/10/12 and the laceration was closed with sutures. He was released back to work full duty at this evaluation.
Employee's pickup truck made contact with a 240 ton production truck. As a result, employee was trapped in vehicle. He was removed and is currently hospitalized.
EE jumped up to tighten the guide roller safety clamp and as he jumped into position, his right thumb was caught in the new belt and roller guide.

As he stepped down from the tool shelf of the man lift, the man lift shifted from his movement and his left thumb was pinched between the man lift railing and the bracket. The employee was evaluated by a physician and dermabond was used to close the wound.
16 ft piece of rigged roller track rotated forcing employee backward, striking back of head/neck against roller track behind him.
The driver of the mine water truck had stopped in the middle of the road to advise the GSSC patrol vehicle that there was brush on the front of the vehicle. The GSSC patrol officer was looking at another area when he returned his eyes forward he had struck the rear of the water truck.
An employee alleges that he was jostled in his truck and jarred his neck. He was evaluated on 4-29-10 by a Physician; Physical Therapy was ordered and he began restricted duty.

Using a Marklift to perform electrical wire work he was feeding wire, installing covers and hanging clamps. He went to move the lift when it shifted. He let go of the control but the brakes did not set immediately. The lift continued to roll down the slanted floor and pinned employee. He was treated for chest and back soreness. He was prescribed Physical Therapy on 1/9/07.
While sliding a liner bar off of a pallet onto a forklift, the liner fell and it hit his foot.
THE FUEL TRUCK WAS INVOLVED IN A ROLL OVER ACCIDENT. WHEN HE RELEASED HIS SEAT BELT HE FELL 4-5 FEET WITHIN THE CAB SUSTAINING A FRACTURED THORACIC SPINE.

BACKING HAUL TRUCK UNDER SHOVEL- WAS JOLTED IN TRUCK BY CHUNKS BEING DROPPED IN HER TRUCK BOX- SHE FELT SUDDEN PAIN IN HER LEFT SHOULDER. SHE CONTINUED TO HAUL. AT FOLLOWING SHOVEL SHE BACK- ED INTO SHOVEL BUCKET WITH HER TRUCK, BRACED HERSELF BUT WAS THROWN AGAINST THE STEERING WHEEL. SHE EXPERIENCED STABBING PAIN IN SHOULDER.
EE WAS DOING HIS CONVEYOR CHECKS ON THE 2A CONVEYOR. WHILE DRIVING THE CART DOWN THE RAMP HE STRUCK A CEMENT/CONCRETE SUPPORT BEAM. HE WENT FORWARD IN THE CART & STRUCK HIS KNEES INTO THE CART HOUSING RESULTING IN A LACERATION TO HIS KNEE WHICH REQUIRED STITCHES.
EE WAS HAULING FROM NEW CRUSHER SURGE PILE WITH 5045 TRUCK. ON HIS WAY TO THE EAST SIDE OF THE CRUSHER EE DROVE THE LEFT FRONT TIRE UP ON TOP OFBERM ON THE SE CORNER OF CRUSHER CAUSING TRUCK T O FLIP ON IT'S RIGHT SIDE. EE DID NOT HAVE A SEAT BELT ON AND FELL THRU WINDOW LANDING ON HANDRAIL. ON 3-23-01 HE WAS DIAGNOSED WITH A FRACTURED R THUMB AND IS ON RESTRICTED DUTY.
EE WAS EXITING THE LOADER TO SHUT THE OVERHEAD DOOR. WHILE EXITING THE LOADER, HE REALIZED IT WAS STILL RUNNING SO HE TRIED TO RE-ENTER TO SHUT OFF THE LOADER AND HIS FOOT STEPPED ON THE CONTR OL PEDAL FOR THE BUCKET. HIS FOOT WAS PINCHED BETWEEN THE BUCKET & LOADER RESULTING IN A CONTUSION TO THE ANKLE.
SHOULDER OVEREXERTION AND IRRITATION DUE FREQUENT OPERATION OF 90 TON TRUCK. THE STEERING WHEEL IS MUCH SMALLER THAN A 170 TRUCK.
INJURED WAS DRIVING OFF OF A WASTE DUMP IN HIS FUELER TRUCK WHEN HE HIT SOME BUMPS AND STRUCK HIS HEAD ON THE BACK OF THE CABL. PRESCRIBED PHYSICAL THERAPY 8-28-00.
EE WAS A PASSENGER SITTING ON THE SEAT OF THE GOLFCART. HIS PARTNER (DRIVING) TURNED THE CORNER UNEXPECTEDLY & EE SLID & ALMOST FELL OUT OF THE CART, INJURING HIS ANKLE WHEN HE ATTEMPTED TO ST ABLIZE HIMSELF WITH HIS FOOT. ON 2-22-01 HE WAS PRESCRIBED PHYSICAL THERAPY ON THE ANKLE.
EMPLOYEE WAS RIDING IN A PASSENGER VAN GOING TO THE PIT TO THE PRODUCTION EQUIPMENT HE WAS OPERATING. HE WAS SEATED IN THE LAST SEAT OVER BACK TIRE OF THE VAN WHEN IT HIT A POTHOLE AND EMPLOY EE WHO WAS WEARING HIS SEATBELT STRUCK HIS HEAD ON THE CEILING OF THE VAN. VAN SPEED WAS 5 TO 10MPH.
INJURED WAS OPERATINGA LOADER AND DID NOT NOTICE A BURIED CHUNK. WHEN THE LOADER BUCKET STRUCK THE CHUNK HE BUMPED HIS ELBOW ON THE CAB. HE WASPRESCRIBED PHYSICAL THERAPY 3-7-00.
