DRIVING TRUCK TO MOVE BOULDERS ALL DAY WITH WINDOWS DOWN, HE STARTED FEELING SICK IN THE AFTERNOON. THERE WAS A HOLE IN THE FLOOR BOARD OF THE TRUCK. HE WENT TO THE HOSPITAL BUT THEY DID NOT DO A BLOOD TEST TO SEE IF HE DID HAVE CARBON MONOXIDE POISONING. THEY DID A CAT SCAN TO RULE OUT TUMOR.

EE WAS WRITING PRODUCT NAME AND WEIGHT ON PALLETS AND A COWORKER IN THE FRONT-END LOADER WAS PICKING UP PALLETS AND MOVING THEM FOR AN ORDER. THE COWORKER CAME IN TO PICK UP A PALLET AND THE FORKS WERE A LITTLE OFF TRACK AND PUSHED THE PALLET AND ROCK INTO EE, HITTING HIM IN THE RIGHT ARM AND BACK.

MINER TRAVELING DOWN A RAMP IN A F.E.L., HE LOST CONTROL AND WENT OVER A BERM, SLID 18'DOWN THE SIDE OF THE RAMP AND ROLLED ONTO THE TOP. THE MINER'S LEG WAS CAUGHT UNDER THE SEAT, TWISTING TH E LEG AND DISLOCATING THE HIP. CONDITIONS WERE CLEAR AND DRY.

HE PLACED HIMSELF IN HARMS WAY BETWEEN A MOVING LOADER AND STATIONARY FLAGSTONE CONVEYOR; LOADEROPERATOR'S FOOT SLIPPED OFF BRAKE AND HIT EXCELERATOR. THE LOADER LEAPT FORWARD, PINNING VICTIM BETWEEN THE CONVEYOR AND LOADER.
