Operator stoppedshort;couplers misaligned.IE entered red zone without protection, violating CS-4(Red Zone) and CS-27 (Adjusting mismatched coupler with or without a device(SafeTRAX) to line couplers. Used their foot to align couplers. Operator continued shoving; IE foot was caught and pinched between.

While riding the GR2 Man Lift, a piece of debris flew into the employee's eye. Employee was wearing proper eye protection. Went to eye doctor, material was removed, and was prescribed medical eye drops.
Load out operator traveling to dodge parking and struck a parked lowboy supply trailer. Results were hairline fracture in the upper tarsal bone in EE's right foot.

EMPLOYEE WAS CLEANING BELTLINE & GOT HIS RIGHT HAND CAUGHT BETWEEN THE BELT & ROLLER.
Employee was loading shuttle car. When shuttle car was finished loading and started to leave room, it hooked a rope that is used to drag miner cable. Cable pulled tight knocking employee to the ground. He did not miss any work but was on restricted duty.

EE WAS RIDING IN THE BACK OF MANTRIP ON THE RIGHT SIDE OF MANTRIP. AS THE MANTRIP PASSED THE H09N INTERSECTION, EE STRUCK HIS HEAD ON A 4" STEEL WATER LINE. KNOCKING HIS UNCONSCIOUS.

EE WAS IN PANEL L19 AS A UTILITY FOREMAN. HE HAD PARKED HIS JEEP IN FIRST XCUT IN PANEL, CREW WAS DOING A PULL BACK, ANOTHER EE WAS OPERATING A LOWTRAC, MOVING A TRASH HOPPER, DID NOT SEE FORE MAN IN FRONT OF HIM & RAN INTO HIM. PINNED HIM BETWEEN TRASH HOPPER & JEEP.
EE WAS DRIVING #4A SHUTTLE CAR IN PANEL SB01, TOWARD THE MINER WHEN HE HIT A BUMP OR DIP IN ROADWAY AND IT BOUNCED HIM INTO CANOPY OF SHUTTLE CAR. EE WORKED UNTIL 2-12-02 + ACCIDENT WAS STILL UNDER INVESTIGATION.
