INJURED WAS REMUCKING LOWER MUCK BAY AND LOADING TRUCK. HE WAS HEADED UP RAMP FROM MUCK BAY WITHA LOADED BUCKET WHEN ALL OF A SUDDEN THE LIGHTS AND POWER TO MACHINE FAILED. BRAKES DID NOT SET AND HE WENT DOWN THE RAMP PUMPING THE FOOT BRAKETHE MACHINE FAILED TO FUNCTION PROPERLY AND INJ URED FINALLY HIT A WALL AND STOPPED.

EMPLOYEE WAS TRAVELING TO WORK PLACE. HE WAS TRAVELING OVER A BNAD SPOT IN THE ROADWAY AND HE LOST CONTROLL HITTING THE RIB AND TIPPING THE TRACTOR OVER ON ITS SIDE. WHEN TRACTOR STARTED TO TIP HE JUMPED OFF AND HE HIT HIS HEAD AND TURNED HIS WRIST WRONGE.
