The operator of HT487 was hauling loads out of the 98 shovel for a majority of the night. The operator believed the floor of the pit was rough and while spotting experienced some neck and back pain.
Sandvik Scoop LH02 operator was mucking in 3250 OD25. As the scoop was moving around a corner the operator was leaning forward with their head turned to get better visibility. The scoop hit a bump causing the operator's head, above the right eye, to contact the window, the operator briefly lost consciousness.
Around 09:05 a haulage operator was being loaded by a shovel and got jolted from a big rock being dumped into the bed. IP arrived at first aid post at 09:36 and was assessed by an EMT. IP could not move head from side to side without pain. EMT sent the IP to RMMC for higher level of care. Pain is on left side of neck and upper trap.
Operator on the 461 made a series of sharp turns by cranking on the steering wheel when the operator felt a pain in their right shoulder.
At 23:50, IP supervisor called to notify EMT of a minor medical. IP arrived and stated that on 8/5 at 06:30 while riding the shuttle up from Park at Lark, they felt pain and described the feeling like a bee sting in lower left abdomen between left hip and belly button which had been gradually worsening throughout the day. IP was referred to ER for further level of care.
After loading at the ES59, the HT471 operator noticed lower back discomfort. The operator attempted to continue operating to see if the pains would subside. After about a half hour, and the pains continued, the operator reported the pains to their supervisor. The operator was assessed by onsite ERT's, EMT then RMCC where they were released back to duty with restrictions.
HT driver stated their haul truck was getting loaded around 9pm last night and they noted neck and back pain at the time of the load. EE continued driving and had 2 subsequent loads that worsened the pain in low back and neck. Employee stated they had a hard time breathing due to the muscle tightness.
Employee and a geo-tech were pull-testing bolts. A bolt plate was placed beneath one of the legs of the pull tester so the leg would not get trapped in the mesh. Tension was pulled and the bolt passed. The employee unscrewed the nut securing the pull testing machine to the bolt. When removing the nut, the bolt plate became loose and fell 3-4 ft hitting the employee in the lip.
IP stated while they were operating the haul truck, they hit a bump and got pulled by the wheel of the haul truck when they hit the bump. Felt a pain in their shoulder which worsened as they were required to pull hard on the steering wheel to get the truck to move. IP was assessed by EMT and was referred to RMMC.
IP was riding bus back to park at lark when bus turned corner from the mine entrance road and hit a hole causing IP to experience neck pain. IP was assessed by EMT and denied transport to clinic. Follow-up appt on 03.04.25 wrote "avoid neck movements that cause pain". Able to perform all normal duties at a slower pace.
IP was driving haul truck when they hit a pot hole and began to experience neck pain. Was assessed by EMT. IP went to clinic with supervisor for further evaluation. 03.04.25 visit "Avoid neck movements that increase pain. Avoid overhead shoulder movements that increase pain." Worker able to perform all normal duties at a slower pace.
Operator hit a pot hole and began to experience back and hip pain. The operator notified the supervisor the following shift and went to the ER. Employee has been able to perform normal job duties until last follow up appointment on 03/04/2025 where employee was given the following restrictions: Avoid painful low back movements. No lifting, pushing, pulling > 20 lbs.
12.29.24: Operating HT406, hit a few large dips in the roads in multiple areas. Seen by medical and released full duties. 01.27.25: Operating HT op reported back started to hurt. Medical wrote restrictions. Able to perform all normal duties at a slower pace, however.
Incident occurred 11-09-2024, however there were no restrictions until 12-11-2024. Person was driving 941 cable truck through the 71-shovel pit. IP was struck by the knob on the steering wheel on the palm side of left thumb causing swelling and pain. They released IP to go back to work.
Operator noticed an exhaust smell, had some discomfort breathing. Used their inhaler to try and catch their breath. That didn't help so the mayday was called, and medical team administered oxygen to help with shortness of breath.
A haul truck operator felt neck/back discomfort when a transport bus hit a pothole on a haul road. Onsite EMT assessed them, then referred for off-site care. Diagnosis: neck joint/ligament sprain, lower back muscle/fascia/tendon strain. Work restrictions given.
Craftsman was cleaning the truck shop pads with the 0051 Skid Steer. During the process scraping the pad, the bucket of the skid steer contacted a steel grate that was under the mud and dirt. The skid steer came to an abrupt halt. From the jolt, the top of the craftsmen's head contacted the rear-view mirror causing a laceration to their head.
The haul truck slid on the ramp, hitting a soft spot near the base, causing it to tip. The operator, properly restrained, suffered non-life-threatening injuries, with muscle strain in the right hip and left shoulder/arm. Extracted and examined at the emergency department, restrictions were imposed.
A loader collided with a light duty vehicle in the underground in a muck bay, which resulted in the one worker sustaining leg and hand injuries and needing to be extricated from the vehicle. The worker was cared for and then life flighted to Health Care in UT.
Line crew craftsman driving 10 wheel flat bed crew truck to the maintenance shop and hit a pot hole in the road felt momentary twinge on left side of lower back, didn't report. Three days later reported to supervisor wouldn't be in for next shift as quad, leg and back were sore.
While traveling down the Common Access decline with three passengers the driver of the UTV lost brakes. The operator was able to steer the UTV into a pillar and the employee riding in the front passenger seat, while wearing a seat belt, hit Nose, Forehead, and Tooth, also scraped right Hand. Lacerations to the eye brow and nose required sutures to close.
Snow covered concrete blankets were to be removed, the employee was hooking the rigging to the forklift when the bucket moved hitting them on the hard hat.
EE injured during a roll over vehicle incident. Driver stated EE saw something in the road and EE swerved. Pick up left the paved road and ended up in the ditch on the side of the road and rolled on to the driver side.
Haul truck operator made contact with the shovel dipper while backing into spot to be loaded. This impact caused the operator to suffer a neck strain. The operator was placed on restricted duty.
While approaching the dump location the haul truck drove over a rough section on the road causing a jolt which caused a pain in the operators neck and back. The operator was seen by an offsite medical facility. The operator has not been able to return to work due to restrictions given by the physician. The operator was diagnosed with a strain in the upper and lower back.
While operating a front end loader the operator rolled over a rock causing EE to jar back. The employee received physical therapy and was given a full release to work with no restrictions.
Two Employees were working to remove a broken spreader beam from the body screen deck to install a new spreader. EE 1 was on the screen deck cutting the spreader to remove. EE asked EE 2 to come up on the crane after the spreader. When EE 2 came up on the crane the button on the controller got stuck on the up position, crane continued to go up breaking the beam free striking EE 1.
An employee was moving lifters to a cart with a forklift. EE did not lift the forks high enough which caused the lifter to fly up and hit another employee in the lip.
Miner states around 3am EE hit a pot hole. Reported event went home. Upon returning for next shift EE said still sore. Was seen at clinic given ibuprofen FA treatment. The 26th reported still sore was taken back to clinic suggested to see specialist and to start PT treatment
Employee traveling in man bus when it hit a pothole experienced a jolt to EE's back. Taken for care, released with restrictions on lifting and movement until evaluation Thursday April 18, 2019 by EE's doctor.
On March 7th I was made aware that miner alleged they drove a medium vehicle sized water truck on 02/12/2019 & 02/13/2019. EE sought medical attention by their own doctor 03/02/2019 & then reported these allegations 03/07/2019 the date I was made aware and began the investigation. Update: Independent Medical Examiner notified company 6/26/19 that this is a work-related injury.
July 18, 2018 miner called to report a back ache miner had for months. Miner does not know of a triggering event other than it's sore from driving haul truck and wanted to get therapy. Had miner seen by doctor. Miner has degenerative bone issues and is prescribed physical therapy and an MRI. Miner is RTW no restrictions begins PT 7/27/18 MRI and follow up to come.
EE hit a bump in bus, then when lifting wheel chock complained of back ache. Seen by 3 Drs no findings 1st Aid. Took to Spine specialist, diagnosed 10/09/17 issues w/ L4-5. Submitting reporting upon new Dx from update to injury on this date. No restrictions awaiting MRI results has missed no work. Will start physical therapy 10/23/17. 2/9/18 co. made aware of new restriction.
Craftsman struck in lower left leg from wheel chock that was run over and projected into craftsman, in opposite bay at fuel island fracturing leg bewteen knee and ankle
Injured did not report injury until 8/23/16. Initially first aid, but condition has worsened. Haul truck driver reports hit pothole in haul road, injured groin area after impact.
A water truck became stalled and rolled back, contacting a pickup truck's front bumper. As a result of the impact, the driver's head contacted the dash/steering column. This resulted in a minor fracture to a lower back vertebrae. Employee was prescribed days away from work.

Operator was riding in man-bus when rock from passing traffic flipped up and shattered window scratching operator's cheek and causing irritation in left eye. EMT cleaned up cheek saw no foreign object in eye; EE was taken to hospital for further exam. The Dr found two pieces of glass not embedded and flicked them out. Operator completed eye exam returned to work
Haul truck operator reported eye irritation in his left eye. Evaluated by EMT and sent to the KUC clinic where a small metallic object was removed from the middle of his cornea. The operator was given some wetting drops for lubrication and returned to work.
The dozer operator working the Code 25 dump observed a crack. Simultaneously, the operator of haul truck 421 arrived and positioned parallel to the dump approximately 75 feet away from the crest. As the truck operator initiated dumping, another crack propagated, the dump face failed, causing the truck to descend with the dump. The haul truck operator sustained minor injury.
Employee was cleaning the inside of his operator cab when he contacted a sharp edge resulting in a laceration to his right ring finger requiring sutures.
While backing up the Forklift the employee in the man basket was thrown up off the deck, the fall protection stopped the employee brining him back down and the employee lacerated his ear and chin on the man basket hand rail.
Electric shovel was loading haul truck operator was working in when the shovel dipper (full) dropped, impacting the truck. This caused equipment damage and injury to operator.
A haulage truck driver was returning to a dump loaded, when he lost control of the haul truck, hit the berm and tipped the haul truck on its side. As a result of the incident the operator sustained injuries to his torso.
Employee sustained a laceration to the forehead from contact with a fire extinguisher when the haul truck (100 ton) he was operating struck a berm on the haul road.

BRAKE ADJUSTMENT NOT TIG HT ENOUGH. CHOCK SET ON ICY SURFACE. PARKING PROCEDURE-ORDER OF ACTIONS TAKEN. EE WAS IN BACK OF TRUCK; HAVING JUST PLACED MATS IN THE TRUCK. DAMAGES TO TRUCK APPROX $1,000, EE - UNKNOWN.
OPERATING A CAT 793-C HAULAGE TRUCK, LO9ADED, TRAVELING UP-GRADE. TRUCK LOST FORWARD MOTION AND BEGAN TO ROLL BACK. OPERATOR WAS UNABLE TO STOP AND TURNED TOWARD THE HIGH-WALL. THE TRUCK ROLLE D OVER ONTO IT'S TOP.
