OPERATED A LOADER 2-4 HOURS TO OTHER WORK. ON 2-22-93 HE WORKED INSIDE AND TALKED ABOUT LOADER. OTHERS OPERATED THE LOADER SEVEREAL DAYS WITHOUT A COMPLAINT. ON 2-23-93, LAYOFF DUE TO LACK OF WORK HE FILED A CLAIM FOR TIME LOSS DUE TO INJURY FROM LOADER SEAT. THIS CLAIMS IS UNDER INVESTIGATION FOR POSSIBLE FRAUD. HE HAS A ROOFING COMPANY ON THE SIDE.

I NAME WAS EXITING CRESTLINE PIT AFTER GETTING A LOAD OF CONCRETE SAND MIDWAY UP THE HALL ROAD THE SUN ALLEGEDLY BLINDED HIS VIEW CAUSING I NAME TO GO OVER THE BERM WITH FRONT TIRE HE PANICIED & PUT POWER TO THE TRUCK & WENT FURTHER OVER THE BERM & SUBSEQUENTLY ROLLED TRUCK HE WAS WEARING HIS SEAT BELT BUT JUMPED FROM TRUCK & HIT HEAD
